大黄牡丹皮汤灌肠对腹腔镜下胆总管探查术后患者胃肠功能的影响

摘　　要：目的:观察大黄牡丹皮汤灌肠对腹腔镜下胆总管探查术后患者胃肠功能的影响。方法:回顾性分析2018年10月至2020年5月该院收治的98例行胆总管探查术的患者的临床资料,按入院时间顺序分为对照组和观察组各49例。对照组采用肥皂水灌肠,观察组采用大黄牡丹皮汤灌肠,比较两组腹痛程度、中医证候积分和胃肠功能恢复时间。结果:术后24、48、72 h,观察组VAS评分均低于对照组,差异有统计学意义(P<0.05);治疗后,观察组中医证候积分低于对照组,差异有统计学意义(P<0.05);观察组肠鸣音恢复时间、肛门首次排气和首次排便时间均短于对照组,差异有统计学意义(P<0.05)。结论:大黄牡丹皮汤灌肠可降低腹腔镜下胆总管探查术后患者疼痛评分和中医证候积分,缩短胃肠功能恢复时间,其效果优于肥皂水灌肠。Objective:To observe effects of Dahuang Mudanpi decoction enema on gastrointestinal function of patients after laparoscopic common bile duct exploration.Methods:A retrospective analysis of the clinical data of 98 patients,who underwent common bile duct exploration in our hospital from October 2018 to May 2020,was performed.According to the order of admission time,they were divided into control group and observation group,each with 49 cases.The control group was given soapy water enema,while the observation group used Dahuang Mudanpi decoction enema.The degree of abdominal pain,the TCM syndrome score and the gastrointestinal function recovery time were compared between the two groups.Results:24,48,and 72 h after the surgery,the VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After the treatment,the scores of TCM syndromes in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the recovery time of bowel sounds,the first anal exhaust time and the first defecation time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:Dahuang Mudanpi decoction enema can reduce the pain score and the TCM syndrome score of the patients after laparoscopic common bile duct exploration,and shorten the gastrointestinal function recovery time.Moreover,it is superior to soapy water enema.
