摘 要:目的探讨经颅多普勒超声(TCD)在急性脑梗死患者颅内血管狭窄中的诊断价值。方法选取2018年2月—2019年1月序贯住院于本院神经内科的108例急性脑梗死患者,入院48小时内每个患者均行TCD检查,随后并与其MRA检查做比较。结果TCD诊断颅内动脉狭窄263条,其中MCA 140条,ACA 38条,PCA 41条,BA 21条,VA狭窄23条;而在采用MRA诊断方案时,其中的MCA、ACA、PCA依次为146条、36条、45条,BA 24条,VA狭窄27条。TCD诊断颅内血管狭窄敏感度及特异度分为86.0%和96.5%。结论经颅多普勒检查作为一个无创性检查手段,能够通过分析血流动力学改变进行对急性脑梗死患者颅内血管狭窄的筛查,可以早期诊断血管狭窄,并对临床治疗提供帮助。Objective To investigate the value of transcranial Doppler(TCD)in the diagnosis of intracranial vascular stenosis in patients with acute cerebral infarction.Methods From February 2018 to January 2019,108 patients with acute cerebral infarction who were hospitalized in neurology department of our hospital were selected.TCD was performed in each patient within 48 hours after admission,and then compared with MRA.Results There were 263 intracranial artery stenosis diagnosed by TCD,including 140 MCA,38 ACA,41 PCA,21 BA,23 VA stenosis;278 stenosis arteries were detected by MRA,including 146 MCA,36 ACA,45 PCA,24 BA,27 VA stenosis.The sensitivity and specificity of TCD were 86.0%and 96.5%.Conclusion TCD,as a noninvasive method,can screen the intracranial vascular stenosis in patients with acute cerebral infarction by analyzing the hemodynamic changes,diagnose the stenosis early and provide help for clinical treatment.
