摘 要:目的分析呼吸道合胞病毒(RSV)感染患儿的临床特征及免疫功能,为临床抗感染提供指导。方法选取68例呼吸道感染患儿为观察组,74例健康体检儿童为对照组,分析感染患儿免疫球蛋白和T细胞亚群水平。收集呼吸道感染患儿鼻咽抽吸物样本,采用直接荧光定性检测样本中病毒,PCR扩增检测RSV临床分型。结果 68例呼吸道感染患者中,感染呼吸道合胞病毒、副流感病毒、腺病毒、流感病毒、肠道病毒、混合型的患者分别为36、11、9、7、3和2例。36例RSV感染患儿中,A亚型26例,B亚型10例。男性患儿25例,女性患儿11例。0~、1~、3~和6~14岁组患儿检出率分别为36.11%,27.78%、22.22%和13.89%,0~1岁组与其他年龄组相比,差异有统计学意义(χ^(2)=5.5556,P=0.0184)。RSVA和RSVB在不同年龄呼吸道感染患儿中均有分布。春夏秋冬四季患儿RSV检出率分别为30.56%、13.89%、22.22%和33.33%。(χ^(2)=8.0000,P=0.0047)。RSVA和RSVB在各季节中均有流行分布。68例观察组患儿IgG2、IgG4、IgM、IgE水平明显低于对照组(均P<0.05)。T细胞亚群CD3+、CD4+、CD4+/CD8+、CD19+水平均低于对照组,差异均有统计学意义(均P<0.05),提示病毒感染引发患儿机体内免疫功能紊乱。结论儿童呼吸道感染病毒类型主要为呼吸道合胞病毒,冬季是病毒感染的流行季节。积极监测病毒感染情况,对抗病毒治疗具有指导意义。Objective To analyze the clinical characteristics and immune function of pediatric patients with a respiratory tract infection in order to guide the clinical treatment of infection. Methods Sixty-eight children with a respiratory tract infection served as the observation group, and 74 healthy children served as the control group. The levels of immunoglobulins and T-cell subsets in children with an infection were analyzed. Nasopharyngeal aspiration samples from children with a respiratory tract infection were collected, the virus in the samples was determined qualitatively with direct fluorescence, and the clinical typing of respiratory syncytial virus(RSV) was accomplished via amplification with PCR. Results Among the 68 pediatric patients with a respiratory tract infection, 36 were infected with an RSV, 11 were infected with a parainfluenza virus, 9 were infected with an adenovirus, 7 were infected with an influenza virus, 3 were infected with an enterovirus, and 2 had a mixed infection. Of the 36 pediatric patients infected with an RSV, 26 were infected with the RSVA subtype and 10 were infected with the RSVB subtype. Pediatric patients consisted of 25 males and 11 females. The rate of detection was 36.11% in children ages 0-1, 27.78% in children ages 1-3, 22.22% in children ages 3-6, and 13.89% in children ages 6-14. The status of the RSV infection differed significantly between children ages 0-1 and the other age groups(χ^(2)=5.5556, P=0.0184). RSVA and RSVB were distributed in children of different ages with a respiratory tract infection. RSV was detected at a rate of 30.56% in the spring, 13.89% in the summer, 22.22% in the autumn, and 33.33% in the winter. The rate of detection in the winter differed significantly from that in other seasons(χ^(2)=8.0000, P=0.0047). RSVA and RSVB were distributed during all seasons.
