摘　　要：目的观察小针刀松解联合康复训练治疗髂胫束挛缩症的临床疗效。方法回顾性分析自2015-01—2019-12诊治的72例髂胫束挛缩症,37例采用小针刀松解联合康复训练治疗(观察组),35例单纯康复训练治疗(对照组)。治疗后对所有患者进行随访,随访6个月后进行疗效评定,观察指标包括疼痛VAS评分、髋关节活动度(屈曲活动度、内收活动度)、臀肌挛缩症功能量化评分。结果72例均获得至少6个月随访,治疗期间未出现晕针、断针、血肿、感染、神经损伤、皮肤感觉异常改变、髋外侧肌群无力等并发症。观察组治疗后6个月疼痛VAS评分低于对照组,治疗后6个月臀肌挛缩症功能量化评分高于对照组,治疗后6个月髋关节屈曲活动度、髋关节内收活动度高于对照组,差异有统计学意义(P<0.05)。结论小针刀松解联合康复训练治疗髂胫束挛缩症可有效改善下肢功能、缓解疼痛、提高患者生活质量,是一种易操作、微创、经济、有效的治疗方法。Objective To observe the clinical effect of small needle knife release combined with rehabilitation training in the treatment of iliotibial tract contracture.MethodsSeventy-two cases of iliotibial tract contracture treated from January 2015 to December 2019 were retrospectively analyzed.Thirty-seven cases were treated with small needle knife release combined with rehabilitation training(observation group),and 35 cases were treated with rehabilitation training only(control group).All patients were followed up after treatment,and the curative effect was evaluated after 6 months of follow-up.The observation indexes included pain VAS score,hip range of motion(flexion range of motion,adduction range of motion)and quantitative score of gluteal muscle contracture function.ResultsAll the 72 patients were followed up for at least 6 months.There were no complications such as needle syncope,broken needle,hematoma,infection,nerve injury,abnormal skin sensation and weakness of lateral hip muscles.The VAS score of pain in the observation group at 6 months after treatment was lower than that in the control group,the quantitative score of gluteal muscle contracture function at 6 months after treatment was higher than that in the control group,the hip flexion range of motion and hip adduction range of motion at 6 months after treatment were higher than those in the control group,the differences were statistically significant(P<0.05).ConclusionSmall needle knife release combined with rehabilitation training in the treatment of iliotibial tract contracture can effectively improve the lower limb function,relieve pain and improve the quality of life of patients.It is an easy to operate,minimally invasive,economic and effective treatment method.
