摘  要:目的检测老年骨质疏松症患者的血清铁调素、铁蛋白表达水平,并分析二者与老年骨质疏松性骨折的相关性。方法骨折组纳入自2018-06—2019-12确诊的86例老年骨质疏松性骨折,同期存在老年骨质疏松症但未骨折的86例纳入未骨折组。采用酶联免疫吸附试验检测血清铁调素、铁蛋白表达水平。采用Pearson法分析血清铁调素表达水平、血清铁蛋白表达水平与不同部位骨密度的相关性。采用多因素Logistic回归分析老年骨质疏松性骨折发生的影响因素。结果骨折组检测发现血清铁调素表达水平为(70.17±15.06)ng/mL,未骨折组为(109.39±27.28)ng/mL,骨折组低于未骨折组(P<0.05)。骨折组血清铁蛋白表达水平为(275.92±32.08)ng/mL,未骨折组为(182.12±22.43)ng/mL,骨折组高于未骨折组(P<0.05)。血清铁调素表达水平与股骨颈、股骨大粗隆、总髋部、Ward三角区、股骨粗隆间、腰椎部位骨密度呈正相关,而血清铁蛋白表达水平与各部位骨密度呈负相关。多因素Logistic分析结果显示血清铁调素低表达、血清铁蛋白高表达是老年骨质疏松性骨折发生的危险因素。结论老年骨质疏松性骨折患者血清铁调素表达水平明显降低,铁蛋白表达水平显著升高。老年骨质疏松症患者应定期检测血清铁调素、铁蛋白表达水平,以更好地预测骨折发生的风险,进而采取有效的预防措施。Objective To detect the expression levels of serum hepcidin and ferritin in elderly patients with osteoporosis,and to analyze the correlation between the two and elderly osteoporotic fractures.MethodsThe fracture group included 86 elderly osteoporotic fractures diagnosed from June 2018 to December 2019,and the non-fracture group included 86 cases of elderly osteoporosis but without fracture.Enzyme-linked immunosorbent assay was used to detect the expression levels of serum hepcidin and ferritin.Pearson method was used to analyze the correlation between serum hepcidin expression level,serum ferritin expression level and bone mineral density in different parts.Logistic regression was used to analyze the influencing factors of osteoporotic fracture in the elderly.ResultsThe expression level of serum hepcidin in the fracture group was(70.17±15.06)ng/mL,and that in the non-fracture group was(109.39±27.28)ng/mL,the fracture group was lower than the non-fracture group(P<0.05).The expression level of serum ferritin in the fracture group was(275.92±32.08)ng/mL,and that in the non-fracture group was(182.12±22.43)ng/mL,the fracture group was higher than the non-fracture group(P<0.05).The expression level of serum hepcidin was positively correlated with bone mineral density in the femoral neck,greater trochanter,total hip,Ward triangle,intertrochanter,and lumbar spine,while the expression level of serum ferritin was negatively correlated with bone mineral density in each part.Multivariate Logistic analysis showed that low expression of serum hepcidin and high expression of serum ferritin were risk factors for osteoporotic fractures in the elderly.ConclusionThe expression level of serum hepcidin in elderly patients with osteoporotic fractures is significantly reduced,and the expression level of ferritin is significantly increased.The expression levels of serum hepcidin and ferritin should be regularly tested for elderly patients with osteoporosis to better predict the risk of fractures and take effective preventive measur
