托特罗定联合经闭孔尿道中段悬吊术治疗产后压力性尿失禁的效果

摘　　要：目的探究托特罗定联合经闭孔尿道中段悬吊术(TVT-O)治疗产后压力性尿失禁的效果。方法选取2018年1月-2019年12月宁波市鄞州人民医院收治的产后压力性尿失禁患者100例为研究对象,随机分为对照治疗组和联合治疗组,每组各50例。对照治疗组患者采用TVT-O术治疗,联合治疗组采用托特罗定联合TVT-O术治疗。评价两组患者尿失禁频率、尿失禁程度、夜尿次数、尿动力学、尿失禁症状、生活质量、性生活质量、临床疗效及不良事件发生情况。结果联合治疗组患者尿失禁频率、尿失禁程度及夜尿次数均低于对照治疗组,差异均有统计学意义(均P<0.05)。联合治疗组患者控制面积、最大逼尿肌压及最大尿道关闭压均高于对照治疗组,差异均有统计学意义(均P<0.05)。联合治疗组患者ICI-Q-SF评分低于对照治疗组,I-QOL、PISQ-12评分高于对照治疗组,差异均有统计学意义(均P<0.05)。联合治疗组患者治疗总有效率显著高于对照治疗组,差异有统计学意义(P<0.05)。联合治疗组患者不良事件总发生率低于对照治疗组,差异有统计学意义(P<0.05)。结论托特罗定联合TVT-O术对产后压力性尿失禁患者进行治疗,可改善患者尿动力学,改善尿失禁症状,提高生活质量,效果显著。Objective To investigate the effect of tolterodine combined with transobturator vaginal tape inside-out(TVT-O)in the treatment of postpartum stress urinary incontinence.Methods A total of 100 patients with postpartum stress urinary incontinence admitted to Yinzhou People’s Hospital of Ningbo from January 2018 to December 2019 were selected as the research subjects,and randomly divided into control treatment group and combined treatment group,with 50 patients in each group.Patients in the control treatment group were treated with TVT-O,and patients in the combined treatment group were treated with tolterodine combined with TVT-O.The frequency of urinary incontinence,degree of urinary incontinence,nocturia,urodynamics,symptoms of urinary incontinence,quality of life,quality of sexual life,clinical efficacy and the incidence of adverse events in two groups were evaluated.Results The frequency of urinary incontinence,the degree of urinary incontinence and The Times of nocturia in combined treatment group were lower than those in control treatment group,with statistical significance(all P<0.05).The control area,maximum detrusor pressure and maximum closing urethral pressure in combined treatment group were higher than those in control group,with statistical significance(all P<0.05).The ICI-Q-SF score in combined treatment group was lower than that in control treatment group,while the I-QOL and PISQ-12 scores were higher than those in control treatment group,with statistical significance(all P<0.05).The total effective rate in the combined treatment group was significantly higher than that in the control treatment group,with statistical significance(P<0.05).The total incidence of adverse events in the combined treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Tolterodine combined with TVT-O in the treatment of postpartum stress urinary incontinence patients,can improve the patients’urodynamics,improve the symptoms of urinary incontinence,imp
