摘　　要：目的掌握雅安市克山病病情和防治工作组织管理现状,评价病区取得的控制和消除克山病成果。方法2006-2018年在雅安市8个病区县开展13次病例搜索,每县根据病例搜索结果选取1个或1个以上病例较多的乡镇,再从其中选取病例较多的1个村开展重点调查,对照国家《重点地方病控制和消除评价办法》、《四川省克山病控制和消除评价实施方案》评价各县(区)取得克山病病区控制和消除成果情况。采用卡方检验和等级相关进行统计分析。结果截至2018年底,雅安市8个病区县有101个病区乡(镇);2006-2018年所有病区县无急型、亚急型克山病发生,病例搜索出慢型克山病17例,重点调查出慢型克山病10例、潜在型克山病27例,各病区乡(镇)慢型克山病患病率为0.00‰~0.42‰,潜在型克山病检出率为0.00‰~7.14‰,八个县(区)达到克山病消除标准。结论雅安市克山病病情平稳低发,防治工作取得消除成果,但克山病病因不明,病区仍有新病例发现,监测工作需持续开展以发现风险点,持续做好健康教育,维持消除状态。Objective To master the current situation of Keshan disease and the organization and management of prevention and treatment work in Ya′an City,and to evaluate the achievement of control and elimination of the disease in the affected area.Methods A total of 13 case searches were carried out in the eight affected counties/districts from 2006 to 2018.According to the results of case searches,one or more towns with more cases was/were selected from each county/district,and then one village with more cases was selected to carry out the key investigation from each town.The achievement of control and elimination of Keshan disease in each county/district was evaluated according to the National Key Endemic Disease Control and Elimination Evaluation Methods and the Implementation Plan of Keshan Disease Control and Elimination Evaluation in Sichuan Province.Chi-square test and rank correlation were used for statistical analysis.Results By the end of 2018,there were 101 affected towns in 8 affected counties/districts in Ya′an City.From 2006 to 2018,there was no acute or subacute Keshan disease in all the districts or counties.A total of 17 cases of chronic Keshan disease were found in case searches,and 10 cases of chronic Keshan disease and 27 cases of latent Keshan disease were mainly investigated.The prevalence rate of chronic Keshan disease in affected towns was 0.00-0.42‰.The detection rate of latent Keshan disease was 0.00-7.14‰.Eight counties/districts reached the elimination standard of this disease.Conclusion The Keshan disease conditions of Ya′an City were continuously stable and low.The prevention and treatment work of Keshan disease has achieved the success of elimination.However,the cause of the disease is unknown.There were still new cases found in the affected area.Monitoring work should be continuously carried out to find the risk points,health education should be continuously well done,and the state of elimination should be maintained.
