红藤饮联合乌司他丁对重症腹腔感染患者炎症因子的调节作用

摘　　要：目的探究红藤饮联合乌司他丁对重症腹腔感染患者的疗效。方法选择本院收治的腹腔感染患者147例作为研究对象,其中对照组患者72例采用常规抗感染支持治疗,并给予乌司他丁注射液静脉滴注,10万U 2次/d;治疗组患者75例在采用对照组治疗方法的同时给予自拟红藤饮治疗,两个疗程后评估两组患者的疗效。同时酶联免疫吸附法检测治疗前后患者血清中CRP、PCT、TNF-α和IL-6因子水平,RT-PCR法检测患者外周血中TNF-α和IL-6的水平变化,比较两组治疗后各指标差异。结果两个疗程治疗后,自拟红藤饮联合治疗的治疗组总有效率为93.33%,疗效显著优于常规治疗的对照组79.17%;两组APACHE II评分较治疗前均降低(P<0.001),较对照组,治疗组APACHE II评分降低更明显(P<0.001);对照组血清中CRP、PCT、TNF-α因子水平较治疗前显著降低(P<0.05),治疗组血清中CRP、PCT、TNF-α和IL-6因子水平较治疗前同样降低明显(P<0.05),且较对照组差异有统计学意义(P<0.05);同时RT-PCR结果显示对照组TNF-α和IL-6表达水平较治疗组高(P<0.05)。结论红藤饮联合乌司他丁能明显改善重症腹腔感染患者的临床症状,降低炎症因子表达水平,提高患者生存质量。Objective To explore the curative effect of Hongteng Soup combined with ulinastatin on patients with severe abdominal infection.Methods 147 patients with abdominal infection in our hospital were selected as the research objects.72 patients in the control group were treated with routine anti-infection support treatment,and were given ulinastatin injection intravenous drip,with 100000 units twice a day,while 75 patients in the treatment group were treated with self-made Hongteng Soup at the same time as the control group.The curative effects of the two groups were evaluated after two courses of treatments.At the same time,the levels of CRP,PCT,TNF-αand IL-6 were detected by enzyme-linked immunosorbent assay,and the levels of TNF-αand IL-6 were detected by RT-PCR.The differences of the level of each index between the two groups after treatment were compared.Results After two courses of treatments,the total effective rate of the treatment group was 93.33%,which was significantly better than that of the control group(79.17%).The APACHE II score of the two groups was lower than that before treatment(P<0.001),and the APACHE II score of the treatment group was lower than that of the control group(P<0.001).The serum levels of CRP,PCT and TNF-αin the control group were significantly lower than those of before treatment(P<0.05).The serum levels of CRP,PCT,TNF-αand IL-6 in the treatment group were significantly lower than those of before treatment(P<0.05).And there was a significant difference compared with the control group for these markers(P<0.05).RT-PCR results showed that the expressions of TNF-αand IL-6 in the control group were higher than those in the treatment group(P<0.05).Conclusion Self-made Hongteng Soup combined with ulinastatin can significantly improve the clinical symptoms of patients with severe abdominal infection,reduce the expression level of inflammatory factors,and improve the quality of life of patients.
