摘  要:目的分析多配体蛋白聚糖(SDC-1)、缺氧诱导因子(HIF-1α)、颗粒蛋白前体(PGRN)与急性加重期慢性阻塞性肺疾病(AECOPD)患者肺功能及预后的相关性。方法选取2019年1月至2021年1月本院收治的AECOPD患者114例作为观察组,另选取同期体检健康的志愿者48例作为对照组,收集两组临床资料及外周血2 mL,检测血清SDC-1、HIF-1α、PGRN水平,比较两组1秒用力呼气容积占预计值百分比(FEV1%pred)、1秒用力呼气容积/用力肺活量(FEV1/FVC);114例AECOPD患者依据30 d存活情况分为生存组和死亡组,对影响预后的因素进行相关性分析。结果观察组血清SDC-1、HIF-1α、PGRN显著高于对照组(P<0.01),FEV1%pred、FEV1/FVC显著低于对照组(P<0.01);SDC-1与FEV1%pred、FEV1/FVC呈线性负相关(r=-0.635,r=-0.700,P=0.001);HIF-1α与FEV1%pred、FEV1/FVC呈线性负相关(r=-0.849,r=-0.816,P=0.001);PGRN与FEV1%pred、FEV1/FVC呈线性负相关(r=-0.643,r=-0.666,P=0.001);114例AECOPD患者存活72例,病死42例。Logistic分析结果显示,HIF-1α、PGRN是AECOPD预后不良的危险因素(P<0.05)。结论AECOPD患者血清SDC-1、HIF-1α、PGRN高于正常水平,三者与肺功能密切相关,高龄、住院时间延长、肺功能低下,血清HIF-1α、PGRN水平偏高是预后不良的危险因素。Objective To analyze the correlation of syndecan-1(SDC-1),hypoxia-inducible factor(HIF-1α)and progranulin(PGRN)with lung function and prognosis of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 114 patients with AECOPD admitted to the hospital from January,2019 to January,2021 were selected as the observation group,while 48 healthy volunteers were selected as the control group.Clinical data and 2 mL of peripheral blood were collected from the two groups to detect the levels of serum SDC-1,HIF-1αand PGRN.The forced expiratory volume in the first second in percent predicted values(FEV1%pred),and forced expiratory volume in one second/forced vital capacity(FEV1/FVC)ratio were compared between the two groups.All patients enrolled were divided into the survival group and death group according to the survival status within 30 days.Prognostic factors were then analyzed.Results Serum SDC-1,HIF-1αand PGRN levels in the observation group were significantly higher than those of the control group(P<0.01),while FEV1%pred and FEV1/FVC ratio were significantly lower than those of the control group(P<0.01).SDC-1 was negatively correlated with FEV1%pred and FEV1/FVC ratio(r=-0.635,r=-0.700,P=0.001).HIF-1αwas negatively correlated with FEV1%pred and FEV1/FVC(r=-0.849,r=-0.816,P=0.001).PGRN was negatively correlated with FEV1%pred and FEV1/FVC(r=-0.643,r=-0.666,P=0.001).In this study,72 patients survived and 42 cases died.Logistic analysis showed that HIF-1αand PGRN were risk factors for the poor prognosis of AECOPD(P<0.05).Conclusion Serum SDC-1,HIF-1α,and PGRN levels are elevated in patients with AECOPD.These three markers are closely related to lung functions.Advanced age,long hospital-stay,poor lung function,high serum HIF-1αand PGRN levels are risk factors for the poor prognosis.
