多囊卵巢综合征患者不良妊娠结局的影响因素分析

摘　　要：目的分析多囊卵巢综合征(PCOS)患者不良妊娠结局的影响因素。方法43例PCOS患者(观察组)分为观察1组(无不良妊娠结局,28例)和观察2组(有不良妊娠结局,15例),43例正常孕妇为对照组。比较观察组和对照组身体质量指数(BMI)、睾酮、黄体生成素、妊娠期高血压、妊娠期糖尿病等指标;采用多因素Logistic回归分析PCOS患者不良妊娠结局的影响因素。结果观察组BMI、睾酮、黄体生成素、妊娠期高血压、糖尿病发生率均高于对照组(P<0.01或0.05);PCOS患者不良妊娠结局的独立危险因素有BMI≥25 kg/m^(2)、腰臀比≥0.80、睾酮≥45μg/L、妊娠期糖尿病。结论BMI过高、腰臀比过大、睾酮水平升高、妊娠期糖尿病可对PCOS患者妊娠结局产生不良影响。Objective To analyze the influencing factors of adverse pregnancy outcomes(APOs)in polycystic ovary syndrome(PCOS).Methods Forty-three PCOS patients were divided into observation group 1(absent APOs,n=28)and observation group 2(existing APOs,n=15),and 43 normal pregnant women served as control group.Body mass index(BMI),testosterone,luteinizing hormone,and gestational hypertension and diabetes mellitus were compared between two groups.The influencing factors of APOs in PCOS patients were analyzed using multivariate logistic regression analysis.Results BMI,serum levels of testosterone and luteinizing hormone,and gestational hypertension and diabetes were higher in observation group than in control group(P<0.01 or 0.05).The independent risk factors for APOs included BMI≥25 kg/m^(2),waist-hip ratio≥0.80,testosterone≥45μg/L,and gestational diabetes in PCOS patients.Conclusion Abnormal BMI and waist-hip ratio,hypertestosteronemia,and gestational diabetes may have adverse effects on pregnancy outcomes in PCOS patients.
