摘  要:目的探讨系统性红斑狼疮(SLE)患者血清5-羟色胺(5-HT)及其受体[5-羟色胺2A受体(5-HT2AR)和5-羟色胺7受体(5-HT7R)]在CD8+T细胞的表达。方法选取2019年8月至2020年10月在中国医科大学附属第一医院风湿免疫科住院的SLE患者35例及健康对照(HC)者30例,比较两组间血清5-HT水平及CD8+T细胞5-HT2AR mRNA和5-HT7R mRNA表达的差异,并分析5-HT与两种受体的相关性,以及三者与其他临床指标的相关性。结果与HC组相比,SLE组血清5-HT水平、CD8+T细胞5-HT2AR mRNA及5-HT7R mRNA表达均显著减低[(38.02±20.55)μg/L比(179.03±46.48)μg/L,P<0.001;(0.58±0.20)比(1.13±0.25),P<0.001;(0.37±0.24)比(1.11±0.19),P<0.001]。且CD8+T细胞5-HT2AR mRNA及5-HT7R mRNA表达与血清5-HT水平均正相关(r=0.551,P=0.001;r=0.511,P=0.002)。血清5-HT水平与血小板计数、血清C3和C4水平呈显著正相关(r=0.486,P=0.003;r=0.562,P<0.001;r=0.551,P=0.002),与红细胞沉降率(ESR)和SLEDAI评分负相关(r=-0.536,P=0.001;r=-0.565,P<0.001)。SLE组CD8+T细胞5-HT2AR mRNA与C3和C4呈显著正相关(r=0.382,P=0.023;r=0.521,P=0.001);5-HT7R mRNA与C3和C4呈显著正相关(r=0.532,P=0.001;r=0.436,P=0.009),且5-HT7R mRNA与SLEDAI评分呈显著负相关(r=-0.405,P=0.016)。结论5-HT可能通过结合CD8+T细胞5-HT2AR和5-HT7R受体影响CD8+T细胞并介入到免疫系统网络,从而在SLE的发病过程中发挥重要作用,同时为评价SLE疾病活动度提供可能的临床指标。Objective To explore the expression of serum 5-hydroxytryptamine(5-HT)level and its receptors(5-HT2AR and 5-HT7R)in CD8+T cells in systemic lupus erythematosus(SLE)patients.Methods A total of 35 patients with SLE hospitalized in the Department of Rheumatology and Immunology,the First Affiliated Hospital of China Medical University,from August 2019 to October 2020 and 30 healthy controls(HC)were selected.Serum level of 5-HT and the differences in the expression level of 5-HT2AR mRNA and 5-HT7R mRNA in CD8+T cells were compared between SLE group and HC group.The correlation between 5-HT and the two receptors and the correlation between the three and other clinical indexes were analyzed.Results Compared with HC group,5-HT level and CD8+T cell 5-HT2AR and 5-HT7R expression in SLE group were decreased significantly[(38.02±20.55)μg/L vs.(179.03±46.48)μg/L,P<0.001;(0.58±0.20)vs.(1.13±0.25),P<0.001;(0.37±0.24)vs.(1.11±0.19),P<0.001].CD8+T cell 5-HT2AR mRNA and 5-HT7R mRNA expression was positively correlated with serum 5-HT level(r=0.551,P=0.001;r=0.511,P=0.002).Serum 5-HT level of SLE patients was positively correlated with platelet count,C3 and C4 levels significantly(r=0.486,P=0.003;r=0.562,P<0.001;r=0.551,P=0.002),and negatively correlated with ESR and SLEDAI score(r=-0.536,P=0.001;r=-0.565,P<0.001).The 5-HT2ARmRNA level was positively correlated with the levels of C3 and C4(r=0.382,P=0.023;r=0.521,P=0.001);5-HT7RmRNA level was positively correlated with the levels of C3 and C4(r=0.532,P=0.001;r=0.436,P=0.009),and negatively correlated with SLEDAI score(r=-0.405,P=0.016).Conclusions 5-HT may affect CD8+T cells by binding to 5-HT2AR and 5-HT7R receptors of CD8+T cells and thereby intervene in the immune system network,thus playing an important role in the pathogenesis of SLE.Meanwhile,they may provide a new clinical indicator for evaluating disease activity of SLE。
