寰枢椎后路钉棒内固定治疗新鲜Ⅱ型齿状突骨折的疗效

摘　　要：目的探讨寰枢椎后路钉棒内固定治疗新鲜Ⅱ型齿状突骨折的疗效。方法采用寰枢椎后路钉棒内固定治疗10例新鲜Ⅱ型齿状突骨折患者。观察齿状突骨折愈合情况。结果患者均获得随访,时间5~18个月。手术时间90~180 min,术中出血量80~300 ml。术中齿状突骨折复位满意,未发生脊髓、椎动脉损伤或难以控制的静脉丛出血;术后未发生切口感染。患者均成功完成寰枢椎后路钉棒固定,末次随访时骨折均愈合。结论采用寰枢椎后路钉棒内固定治疗新鲜Ⅱ型齿状突骨折临床疗效满意,为二期取出内固定保留寰枢椎旋转功能提供了可能。Objective To investigate the effect of posterior atlantoaxial screw-rod fixation for fresh type Ⅱ odontoid fracture. Methods The 10 cases with fresh type Ⅱ odontoid fractures were treated with posterior atlantoaxial screwrod fixation. Results All cases were followed up for 5 ~ 18 months,The operative duration ranged from 90 min to180 min. The intraoperative blood loss was 80 ~ 300 ml. The satisfactory fracture reduction was achieved during the operation. No spinal cord or vertebral artery injury or uncontrollable venous plexus bleeding or incision infection occurred. All cases got a successful posterior altantoaxial screw-rods fixation and had bony healing at the last follow-up peroid. Conclusions For patients with fresh type Ⅱ odontoid fracture,the posterior atlantoaxial screw-rod fixation has satisfatory effect,which provide the possibility of the second-stage removal of instrument and preserve the rotation function of altanoaxial vertebrae.
