摘　　要：目的探讨直接前方入路半髋关节置换治疗老年股骨颈骨折的近期疗效。方法采用直接前方入路半髋关节置换治疗36例老年单侧股骨颈骨折患者。观察术后并发症发生情况;采用疼痛VAS评分及HHS评分评价临床疗效。结果患者均获得随访,时间6~32个月。术后切口均一期愈合,无感染、脂肪栓塞及假体周围骨折等早期并发症发生;1例发生下肢深静脉血栓,经抗凝治疗3周后血栓消失。髋关节VAS评分、HHS评分末次随访时较术前明显改善(P <0.05)。结论直接前方入路半髋关节置换治疗老年股骨颈骨折的近期疗效较好,中、远期疗效还需更长时间的随访观察。Objective To investigate the short-term effect of the direct anterior approach hemiarthroplasty for treatment of femoral neck fractures in elderly patients. Methods The 36 patients with femoral neck fractures were treated by direct anterior approach hemiarthroplasty. The postoperative complications situation were observed. Curative effect were evaluated by pain visual analogue scale( VAS) and Harris hip scoring( HHS). Results All patients were followed up for 6 ~ 32 months. Primary healing of incision were obtained in all patients,and no infection,fat embolism or periprosthetic fracture occurred,except one case of intermuscular venous thrombosis of lower extremity,which disappeared after anticoagulation therapy for 3 weeks. At the last follow-up,VAS and HHS were significantly improved than the preoperation( P < 0. 05). Conclusions The direct anterior hemiarthroplasty for treating femoral neck fractures in elderly patients has a good short-term effect. The moderate-and long-term efficacy needs long follow-up.
