两种内固定治疗骨质疏松伴锁骨远端骨折的疗效比较

摘　　要：目的比较两种内固定治疗骨质疏松伴锁骨远端骨折的疗效。方法将82例骨质疏松伴锁骨远端骨折患者按照随机抽签法分为观察组(采用锁骨远端微孔锁定钢板内固定治疗,41例)与对照组(采用锁骨远端钩钢板内固定治疗,41例)。记录手术相关指标及术后并发症情况。采用肩关节Constant评分标准评价临床疗效,采用UCLA肩关节评分、ASES评分、肩关节活动度评估肩关节功能恢复情况。结果患者均获得1年随访。手术相关指标、术后并发症发生率两组比较差异均无统计学意义(P> 0.05)。术后UCLA肩关节评分、ASES评分观察组均明显高于对照组(P <0.05);末次随访时,肩关节Constant评分临床疗效优良率、肩关节活动度观察组均明显高(大)于对照组(P <0.05)。结论锁骨远端钩钢板和锁骨远端微孔锁定钢板内固定治疗骨质疏松伴锁骨远端骨折均能获得满意疗效,但锁骨远端微孔锁定钢板内固定疗效更佳,更利于患者肩关节功能恢复。Objective To compare the effect of two kinds of internal fixation in the treatment of distal clavicle fracture in osteoporosis patients. Methods The 82 osteoporosis cases with distal clavicular fracture were divided into observation group( adopting distal clavicular microporous locking plate internal fixation,41 cases) and control group( distal clavicular hook plate internal fixation,41 cases) by random drawing. The related indexes and postoperative complications were recorded. Constant scoring criteria of shoulder joint were used to evaluate clinical efficacy,and the recovery of shoulder joint function was evaluated by the UCLA shoulder joint score,ASES score,and the shoulder joint activity range. Results The patients were followed up for 1 year. There were no significant differences in operation related indicators,incidence rate of postoperative complications between the two groups( P > 0. 05). The UCLA shoulder joint score and ASES score in the observation group were significantly higher than those in the control group( P < 0. 05). At the last follow-up,the excellent and good rate of clinical effect evaluated by Constant score of shoulder joint and the range of motion of shoulder in the observation group were both significantly higher( larger) than those in the control group( P < 0. 05). Conclusions In the treatment of osteoporosis and distal clavicle fracture,distal clavicular microporous locking plate internal fixation and distal clavicular hook plate internal fixation can obtain satisfactory results,but the distal clavicle microporous locking plate has better curative effect,and is more beneficial to the recovery of shoulder joint function in patients.
