摘　　要：目的探讨经Boyd入路切开复位尺骨斜行截骨延长术治疗儿童陈旧孟氏骨折的临床疗效。方法采用经Boyd入路切开复位尺骨斜行截骨延长术治疗26例陈旧孟氏骨折患儿。术后12个月根据Kim评分标准评价肘关节功能。结果患儿均获得随访,时间12~36个月。截骨处均获得骨性愈合。术后12个月根据Kim评分标准评价肘关节功能:优17例,良7例,可2例,优良率为24/26。拆除石膏后发生桡骨头再脱位1例、半脱位1例。2例截骨处延迟愈合,采用管型石膏固定6个月后愈合。均未发生桡神经损伤、感染等并发症。结论经Boyd入路切开复位尺骨斜行截骨延长术治疗儿童陈旧孟氏骨折术后功能恢复好,并发症少,可较快恢复肘关节及前臂功能。Objective To explore the clinical effect of treatment of pediatric old Monteggia fracture with open reduction and ulnar oblique osteotomy lengthening via Boyd approach. Methods The 26 cases of pediatric old Monteggia fracture-dislocations were treated with open reduction,oblique osteotomy lengthening of ulna via Boyd approach. At 12 months postoperation,the Kim score criterion was used to evaluate the eblow joint function. Results All patients were followed up for 12 ~ 36 months. The bony fusion all achieved at the osteotomy site. At 12 months after the operation,according to Kim score criterian,17 cases were excellent,7 cases were good,2 cases were fair,and the excellent and good rate was 24/26. One case of dislocation and 1 case of subluxation occurred after paster removral. Two cases of osteotomy sites had delay fusion,who were treated with plaster fixation for 6 months. All the patients had no radial nerve injury,infection and other complications. Conclusions Open reduction of radial head and ulnar osteotomy lengthening via Boyd approach are used to treat pediatric old Monteggia fracture-dislocations. The function of elbow joint and forearm can be restored quickly after operation,with few complication.
