摘  要:目的探讨有限切开复位髓内钉结合钢丝捆扎内固定治疗股骨转子下骨折的临床效果及手术技巧。方法采用有限切开复位髓内钉结合钢丝捆扎内固定治疗26例股骨转子下骨折患者。记录手术情况,末次随访时采用Harris功能评分标准评价疗效。结果手术时间70~110 min。术中出血量150~350 ml。患者均获得随访,时间4~20个月。术后无切口感染、骨折不愈合、内固定断裂失效等并发症发生。骨折愈合时间为4~6个月。末次随访时,采用Harris功能评分标准评价疗效:优19例,良6例,可1例,优良率25/26。结论有限切开复位髓内钉结合钢丝捆扎内固定治疗股骨转子下骨折具有创伤小、复位简单、固定可靠、骨折愈合率高等优点。Objective To investigate the clinical effect and surgical technique of limited open reduction and intramedullary nail combined with wire binding for treatment of femoral subtrochanter fracture. Methods The 26 cases of femoral subtrochanteric fractures were treated with limited open reduction and internal fixation with intramedullary nail combined with wire binding. The surgery situation was recorded,Harris function criterion was used to evaluate the efficacy. Results The operation duration was 70 ~ 110 min,intraoperative blood loss was 150 ~ 350 ml. All patients were followed up for 4 ~ 20 months. No complications such as incision infection,nounion,fracture of internal fixation and so on occurred. The fracture healing time was 4 ~ 6 months. At the last follow-up,the evaluation result of Harris function score criteria: 19 cases were excellent,6 good,1 fair,with the excellent-good rate of 25/26. Conclusions Limited open reduction and internal fixation with intramedullary nail combined with wire binding for treatment of subtrochanteric fracture has the advantages of less trauma,easy reduction,reliable fixation,and high fracture healing rate.
