听神经病谱系障碍患儿的ASSR特征

摘　　要：目的探讨听性稳态反应(ASSR)在小儿听神经病谱系障碍(auditory neuropathy spectrum disorder,ANSD)患者中的临床意义。方法选取45例(85耳)诊断为ANSD的患儿(观察组),其中40例为双耳ANSD,4例为单耳ANSD,还有1例双侧ANSD患者仅右耳纳入研究(其左耳ABR阈值为80 dB nHL)。选取32例(64耳)诊断为双侧极重度感音神经性聋的患儿为对照组。采用美国GSI Audera对两组患儿进行ASSR测试。对比两组患儿各频率ASSR阈值。结果观察组左耳500、1000、2000和4000 Hz ASSR阈值分别为100.71±15.49、98.81±13.84、96.67±14.42、87.62±13.24d BHL,右耳对应频率ASSR阈值分别为100.70±18.48、101.86±17.82、91.40±17.60、88.26±14.86 dB HL;对照组左耳对应频率ASSR阈值分别为114.52±7.55、114.84±6.66、116.13±5.49、118.06±4.34 dB HL,右耳对应频率ASSR阈值分别为113.87±7.04、113.55±6.50、114.84±7.13、117.16±5.51 dB HL。观察组双耳对应各频率ASSR阈值均明显低于对照组,差异有显著统计学意义(P<0.01)。结论虽然两组患儿的ABR最大强度刺激未能引出反应,但听神经病谱系障碍患儿各频率ASSR阈值明显低于感音神经性聋患儿;ASSR可以检测出听神经病谱系障碍患者的听觉反应阈值,可以反映出这类患者的听觉系统对持续稳定声音的处理能力。Objective To investigate the clinical value of auditory steady-state responses(ASSRs)in children with auditory neuropathy spectrum disorder(ANSD).Methods The observation group selected 45 children(85 ears)who were diagnosed as ANSD,including 4 unilateral ANSD and 1 bilateral ANSD(ABR threshold of left ear was 80 dB nHL).32 children(64 ears)with bilateral extremely severe sensorineural hearing loss were selected as the control group.The ASSR test was conducted by GSI Audera system,and the ASSR thresholds of each frequency were compared between the two groups.Results The thresholds of observation group in left ear were 100.71±15.49,98.81±13.84,96.67±14.42,87.62±13.24 dB HL at 500,1000,2000 and 4000 Hz,right ears were 100.70±18.48,101.86±17.82,91.40±17.60,88.26±14.86 dB HL at the frequencies;The thresholds of control group in the left ear were 114.52±7.55,114.84±6.66,116.13±5.49,118.06±4.34 dB HL,the right ears were 113.87±7.04,113.55±6.50,114.84±7.13,117.16±5.51 dB HL.The thresholds of the observation group were significantly lower than those of the control group,and the difference was statistically significant(P<0.01).Conclusion Although ABRs were absent in ANSD children,they showed some responses of ASSR.So ASSR can detect the auditory response threshold in children with ANSD and may reflect the ability of the auditory system.
