网格化电凝棒烧灼并剥除壁层胸膜治疗自发性气胸的临床疗效

摘　　要：目的探讨网格化电凝棒烧灼并剥除壁层胸膜治疗自发性气胸的临床效果。方法回顾性分析盐城市大丰人民医院胸外科从2015年1月—2019年12月收治的自发性气胸100例患者资料,在肺大疱切除术的基础上,对比分析网格化电凝棒烧灼联合剥除壁层胸膜术(A组=50例)、胸膜摩擦术(B组=50例)治疗效果。结果A组手术时间、术后疼痛评分、引流胸液量、肺持续漏气时间、引流留置时间分别为(101.56±15.99)min、(5.19±1.33)分、(781.25±229.26)mL、(1.25±0.93)d、(2.94±0.68)d,与B组比较,差异有统计学意义(P<0.001);A组复发率为0.00%,显著低于对照组的32.00%,比较差异有统计学意义(χ^(2)=19.048,P<0.001)。结论网格化电凝棒烧灼并剥除壁层胸膜术联合治疗自发性气胸效果确切,可减少术后复发率。Objective To investigate the clinical effect of cauterizing and removing the parietal pleura with a gridded coagulation rod in the treatment of spontaneous pneumothorax.Methods 100 patients with spontaneous pneumothorax in Department of thoracic surgery,Dafeng people's Hospital of Yancheng City from January 2015 to December 2019 were retrospectively analyzed.Based on bullectomy,the therapeutic effects of grid electrocoagulation rod cauterization combined with parietal pleura peeling(group A=50 cases)and pleural friction(group B=50 cases)were compared and analyzed.Results The operation time,postoperative pain score drainage pleural fluid volume,continuous lung leakage time,and drainage indwelling time in group A were(101.56±15.99)min,(5.19±1.33)points,(781.25±229.26)mL,(1.25±0.93)d,(2.94±0.68)d,the difference was statistically significant compared with group B(P<0.001);the recurrence rate of group A was 0.00%,which was significantly lower than 32.00%of the control group,the difference was statistically significant(χ^(2)=19.048,P<0.001).Conclusion The combined treatment of spontaneous pneumothorax with cauterization and parietal pleura stripping with a grid electrocoagulation rod is effective and can reduce the recurrence rate after surgery.
