循证护理配合延续性护理对膀胱全切原位回肠新膀胱术患者自护能力及生活质量的影响

摘　　要：目的循证护理配合延续性护理在膀胱全切原位回肠新膀胱术中的干预效果,观察患者自护能力和生活质量的改善情况。方法方便选取该院2018年1月—2019年11月进行膀胱全切原位回肠新膀胱术的患者共60例,对照组30例采用常规护理,观察组30例采用循证护理配合延续护理,采用自我护理能力测量量表(exerciseofself-care agencyscale,ES-CA)进行评价;采用简易量表(SF-36)进行生活质量评价。统计两组患者的出血量、手术时间、瘘口愈合时间和住院时间。结果观察组自护能力优良率(90.00%)显著高于对照组(66.67%),差异有统计学意义(χ^(2)=4.812,P<0.05)。两组患者治疗后生活质量各项评分较治疗前升高,差异有统计学意义(P<0.05),观察组治疗后的生活质量各项评分较对照组升高更为明显,差异有统计学意义(P<0.05)。观察组出血量、手术时间、瘘口愈合时间和住院时间均少于对照组,差异有统计学意义(t=4.499、3.561、15.540、14.290,P<0.05)。结论循证护理配合延续性护理在膀胱全切原位回肠新膀胱术中可有效地改善患者自护能力和生活质量。Objective Intervention effect of evidence-based nursing combined with continuation nursing in total cystectomy in situ ileal neobladder to observe the improvement of patients'self-care ability and quality of life.Methods It is convenient to select 60 patients who underwent total cystectomy in situ ileal neobladder in the hospital from January 2018 to November 2019 were enrolled.30 patients in the control group received routine care,and 30 patients in the observation group received evidence-based care combined with continued care.The self-care ability measurement scale(exerciseofself-care agencyscale,ES-CA)was used to evaluate;the simple scale(SF-36)was used to evaluate the quality of life.The bleeding volume,operation time,fistula healing time and hospital stay of the two groups of patients were counted.Results The excellent and good rate of self-care ability of the observation group(90.00%)was significantly higher than that of the control group(66.67%),and the difference was statistically significant(χ^(2)=4.812,P<0.05).The quality of life scores of the two groups of patients after treatment were higher than before treatment,and the difference was statistically significant(P<0.05).The quality of life scores of the observation group after treatment increased more significantly than those of the control group,and the difference was statistically significant(P<0.05).The amount of bleeding,operation time,fistula healing time and hospital stay in the observation group were less than those in the control group,and the difference was statistically significant(t=4.499,3.561,15.540,14.290,P<0.05).Conclusion Evidence-based nursing combined with continuous nursing can effectively improve the self-care ability and quality of life of patients during total cystectomy in situ ileal neobladder.
