社区慢性阻塞性肺疾病患者共患肺外疾病用药信息分析

摘　　要：目的:分析社区慢性阻塞性肺疾病(COPD)患者共患肺外疾病的不合理用药信息。方法:分析2017年6月-2019年9月在门诊和合理用药咨询的社区COPD患者共患肺外疾病的相关用药资料及相关合理用药药学服务指导。结果:共汇总213例患者信息,其中男122例(57.28%),女91例(42.72%);年龄(68.52±15.71)岁,病程(7.32±3.16)年。共患一种肺外疾病62例(29.11%),两种疾病91例(42.72%),三种疾病49例(23.00%),共患四种疾病及以上11例(5.16%),共患三种以上疾病者多重用药情况达100.00%。不合理用药状况以长期固定应用某种药物22.03%、不按时用药15.25%,和不遵医嘱不按剂量用药14.41%比例较高。估计可能存在药物不良反应情况以心律失常11.90%与肝功能损害11.90%占首位。患者要求接受医生有针对性的医疗指导和相关健康教育的愿望较迫切。结论:社区COPD患者共患肺外疾病的不合理用药现象较明显,加强相应的药学服务干预颇为重要。Objective:To analyze the irrational drug use information of COPD patients with co-occurrence of extra-pulmonary diseases in the community.Methods:From June 2017 to September 2019,the data of COPD patients with extra-pulmonary diseases and the guidance of rational drug use pharmaceutical service in outpatient department and community consultation were analyzed.Results:A total of 213 patients were collected.Of these,122 were males(57.28%)and 91 were females(42.72%).The generation was(68.52±15.71)years old,the duration was(7.32±3.16)years.62 cases(29.11%)suffered from one extra-pulmonary disease,two extra-pulmonary diseases,91 cases(42.72%),three extra-pulmonary diseases,49 cases(23.00%),four extra-pulmonary diseases and more,11 cases(5.16%).A total of more than three extra-pulmonary diseases with multiple drug use reached 100%.Long-term fixed application of a certain drug was 22.03%.Did not take the medicine on time was 15.25%,non-compliance with doctor's advice and non-dose medication was 14.41%.The possible adverse drug reactions was estimated to be 11.90%of arrhythmia and 11.90%of liver function damage.Patients required targeted medical guidance and related health education.Conclusion:Unreasonable drug use in community COPD patients with extra-pulmonary diseases is obvious,so it is important to strengthen pharmaceutical service intervention.
