摘　　要：目的:分析ST段抬高型心肌梗死(STEMI)伴急性肝损害的病例,探讨临床诊断治疗意义。方法:通过心内科CCU住院病例资料筛选,对临床STEMI伴急性肝损害的临床病例进行分析。结果:丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)与白细胞计数(WBC)、脑钠肽(BNP)、心肌肌钙蛋白I(cTnI)等升高均存在相关性。结论:STEMI伴急性肝损害,经经皮冠状动脉介入治疗(PCI)或未经PCI治疗,根据临床病例分析,STEMI患者女性、心力衰竭、高血压、糖尿病易并发急性肝损害,而且均有不同程度心力衰竭。AST、ALT、WBC与cTnI、BNP等升高曲线基本相一致。及时行PCI术,开通闭塞血管,恢复心肌供血,降低再灌注损伤,减轻心脏负荷,预防急性肝损害对改善心力衰竭预后有着重要作用。Objective:To analyze the cases of ST-segment elevation myocardial infarction with acute liver damage and discuss the clinical diagnosis and treatment significance.Methods:The clinical cases of ST-segment elevation myocardial infarction with acute liver damage were analyzed by screening the data of inpatient CCU in cardiology.Results:There was correlation between AST,ALT and the increase of WBC,BNP,cTnI.Conclusion:ST-segment elevation myocardial infarction with acute liver damage,after PCI or without PCI treatment,according to the clinical case analysis,STEMI patients with females,heart failure,hypertension and diabetes are prone to acute liver damage,and have varying degrees of heart failure.The elevation curves of AST,ALT,WBC and cTnI,BNP are basically consistent.Implementing PCI in time,opening occluded blood vessels,restoring myocardial blood supply,reducing reperfusion injury,reducing cardiac load and preventing acute liver damage play an important role in improving the prognosis of heart failure.
