骨髓细胞形态学联合骨髓病理活检在血液病诊断中的临床应用价值

摘　　要：目的:分析骨髓细胞形态学联合骨髓病理活检在血液病诊断中的临床应用价值。方法:选取2018年3月-2020年9月各类血液病患者685例,均进行骨髓抽吸涂片和骨髓病理活检同步法取材。对比两种检查方法在各项血液病中的诊断准确率。结果:在骨髓纤维化、骨髓转移癌、再生障碍性贫血、骨髓增生异常综合征、多发性骨髓瘤、非霍奇金淋巴瘤中,骨髓病理活检诊断准确率高于骨髓细胞形态学,差异有统计学意义(P<0.05);在急性淋巴细胞白血病、急性髓系细胞白血病、巨幼细胞贫血、血小板减少性紫癜、慢性粒细胞白血病、真性红细胞增多症、缺铁性贫血、原发性血小板增多症中,骨髓细胞形态学诊断准确率高于骨髓病理活检,差异有统计学意义(P<0.05)。结论:骨髓细胞形态学联合骨髓病理活检,二者同步检查可起到互补作用,提高血液疾病诊断准确率。Objective:To analyze the clinical application value of bone marrow cell morphology combined with bone marrow pathological biopsy in the diagnosis of hematological diseases.Methods:From March 2018 to September 2020,685 patients with various types of blood diseases were selected,and the bone marrow aspiration smears and bone marrow pathological biopsy were obtained simultaneously.We compared the diagnostic accuracy rates of the two inspection methods in various blood diseases.Results:In bone marrow fibrosis,bone marrow metastatic carcinoma,aplastic anemia,myelodysplastic syndrome,multiple myeloma,non-Hodgkin's lymphoma,the diagnosis accuracy of bone marrow pathological biopsy was higher than that of bone marrow cell morphology,the difference was statistically significant(P<0.05).In acute lymphoblastic leukemia,acute myeloid leukemia,megaloblastic anemia,thrombocytopenic purpura,chronic myeloid leukemia,true polycythemia,iron deficiency anemia and primary thrombocytosis,the diagnostic accuracy of bone marrow cell morphology was significantly higher than that of bone marrow pathological biopsy,the difference was statistically significant(P<0.05).Conclusion:Bone marrow cell morphology combined with bone marrow pathological biopsy,the two simultaneous examinations can play a complementary role and improve the accuracy of the diagnosis of blood diseases.
