经腹联合经阴道彩色多普勒超声在子宫疤痕妊娠临床诊断中的价值探讨

摘　　要：目的:探讨经腹联合经阴道彩色多普勒超声在子宫疤痕妊娠诊断中的临床应用价值。方法:选取2010年3月-2020年10月广东省东莞市石碣医院收治的57例疑似子宫疤痕妊娠患者作为本次研究对象,均予采用经腹部超声检查和经阴道超声检查。将术后确诊的结果作为金标准,比较经腹部超声检查和经腹部超声联合经阴道超声检查的诊断结果,分析各自的灵敏度、特异度、误诊率及漏诊率。结果:经腹彩超检查提示子宫疤痕妊娠44例,诊断符合率为66.67%;经腹联合经阴道彩色多普勒超声检查提示子宫疤痕妊娠54例,诊断符合率为85.97%。相比于经腹部超声检查,选择经腹联合经阴道彩色多普勒超声可提高对于子宫疤痕妊娠诊断的准确率,两者的差异有统计学意义(P<0.05)。经腹部超声联合经阴道超声检查的灵敏度比经腹部超声检查高,且误诊率明显低于后者,差异均有统计学意义(P<0.05)。而两种检查的特异度和漏诊率之间比较,差异不明显(P>0.05)。结论:经腹部超声联合经阴道超声检查对子宫疤痕妊娠患者的诊断正确率、敏感度更高,漏诊率更低,为诊断疾病、制定合理的治疗方案以及对患者病情的评估提供了可靠的依据。Objective To evaluate the clinical value of transabdominal combined with transvaginal color Doppler ultrasonography in the clinical diagnosis of uterine scar pregnancy.Methods A total of 57 patients with suspected uterine scar pregnancy treated in Shijie Town Hospital of Dongguan City,Guangdong Province from March 2010 to October 2020 were examined by transabdominal ultrasound and transvaginal ultrasound respectively.Taking the postoperative diagnosis as the gold standard,the diagnostic results of transabdominal ultrasound and transabdominal ultrasound combined with transvaginal ultrasound were compared,and their sensitivity,specificity,misdiagnosis rate and missed diagnosis rate were analyzed.Results Transabdominal color ultrasonography revealed 44 cases of uterine scar pregnancy,with a diagnostic coincidence rate of 66.67%.Transabdominal combined with transvaginal color Doppler ultrasound revealed 54 cases of uterine scar pregnancy,with a diagnostic coincidence rate of 85.97%.The accuracy of transabdominal combined with transvaginal color Doppler ultrasound in the diagnosis of uterine scar pregnancy was higher than that of transabdominal ultrasound,and the difference was statistically significant(P﹤0.05).The sensitivity of transabdominal ultrasound combined with transvaginal ultrasound was significantly higher than that of transabdominal ultrasound,and the missed diagnosis rate of transabdominal ultrasound combined with transvaginal ultrasound was significantly lower than that of transabdominal ultrasound(P﹤0.05).There was no significant difference in specificity and missed diagnosis rate between the two methods(P﹥0.05).Conclusion the diagnostic accuracy and sensitivity of transabdominal ultrasound combined with transvaginal ultrasound are higher and the rate of missed diagnosis is lower in patients with uterine scar pregnancy,which provides a reliable basis for the diagnosis of diseases,the formulation of reasonable treatment plan and the evaluation of patients'condition.
