摘　　要：目的探讨“阶梯式倒U形”生物束带(HADM)阴道紧缩术的术式改进及其临床效果。方法从本手术项目组2020年4月~2021年4月期间阴道松弛求美就医者中,随机选择35例作为观察对象;由同一手术项目组操作,应用同一条束带,在“阶梯式倒U形”术式基础上,增加会阴体部“菱形”设计,同时收紧阴道腔、阴道口和加强会阴体。观察手术时间、术后阴道腔及阴道口收紧情况、会阴体高度变化、并发症以及术后3月求美者对手术满意度。结果本组35例II级阴道松弛者接受了改良型“阶梯式倒U形”生物束带阴道紧缩术;平均手术时间35 min;术后阴道腔(中下段)和阴道口均有明显收紧;平均会阴体高度术前1.5 cm,术后3.5 cm,平均抬高2 cm。术后均未发生直肠损伤、血肿、感染等。术后3月随访,33求美者对手术效果满意,满意率达94.3%。结论改良型“阶梯式倒U形”生物束带阴道紧缩术增加了会阴体部“菱形”设计,在原术式缩窄阴道腔的基础上,加强了对阴道口的收紧和会阴体的修复,具有可操作性强、临床效果突出等优势,值得临床推广应用。Objective To explore the surgical improvement of vaginal tightening in staged inverted U shape operated by biological band(human acellular dermal matrix,HADM).Methods 35 cases were randomly selected as the observation objects among cases of vaginal relaxation from April 2020 to April 2021.The cases were performed by the same surgical team.In order to tightening the vagina cavity,vagina mouth and strengthening the perineum,new design like diamond in perineum based on staged inverted U shape was applied by the same band.Observe the operation time,vaginal cavity and vaginal mouth tightening,perineal height,complications,and patient satisfaction with the surgery after 3 months of the operation.Results 35 cases of vaginal relaxation II underwent modified vaginal tightening in staged inverted U shape operated by biological band.The average surgical time was 35min.Both the vaginal cavity(middle and lower section)and the vaginal mouth were tightened significantly after surgery.The average elevation of perineal height was 2cm from preoperative 1.5cm to postoperative 3.5cm.No rectal injury,hematoma or infection occurred after surgery.Follow-up was conducted in 3 months after operation,and 33 patients were satisfied with the operation effect,and the satisfaction rate reached 94.3%.Conclusion The modified biological band vaginal tightening in staged inverted U shape operated added the diamond design of the perineum in order to tightening vaginal mouth and strengthening perineum on the basis of the narrowed vaginal cavity resulted from original operation.It was suggested that the modified operation has strong maneuverability and high satisfaction of beauty seekers,which is worthy of clinical application.
