磁共振胰胆管水成像与内镜下逆行胰胆管造影术临床应用的比较

摘　　要：目的探究磁共振胰胆管水成像与内镜下逆行胰胆管造影术的临床应用价值。方法选取本院2014年6月至2016年6月收治的疑似胰胆疾病的患者50例作为研究对象,患者均进行ERCP检查(内镜下逆行胰胆管造影术)、MRCP检查(磁共振胰胆管水成像),比较两种检查手段诊断特异度、准确性。结果以手术与病理结果为金标准,50例患者中胆道结石患者23例,胆道重度梗阻10例,非特异性胆道扩张8例,慢性胰腺炎5例,胆囊息肉2例,无病变2例;MRCP特异性为100%、准确度为80%,ERCP特异性为100%、准确度为92%,两种检查方式比较差异均无统计学意义。结论MRCP检查在诊断胰胆疾病方面与ERCP具有相同的价值,两种检查手段各有优势,其中ERCP诊断特异度、准确性均较高,临床需结合患者具体情况合理选择诊断方式。Objective To explore the clinical application value of magnetic resonance cholangiopancreatography and endoscopic retrograde cholangiopancreatography.Methods 50 patients with suspected pancreaticobiliary disease admitted to our hospital from June 2014 to June 2016 were selected as the research subjects.All patients underwent ERCP(endoscopic retrograde cholangiopancreatography)and MRCP(magnetic reso-nance cholangiopancreatography).The diagnostic specificity and accuracy between the two inspection methods were compared.Results With surgi-cal and pathological results as the gold standard,among the 50 patients,23 cases had biliary stones,10 cases had severe biliary obstruction,8 cases had non-specific biliary dilatation,5 cases had chronic pancreatitis,2 cases had gallbladder polyps,and 2 cases had no lesions;The specificity of MRCP was 100%,the accuracy was 80%,the specificity of ERCP was 100%,and the accuracy was 92%;there was no statistically significant differ-ence between the two inspection methods.Conclusion MRCP examination has the same value as ERCP in the diagnosis of pancreaticobiliary dis-eases.Both examination methods have their own advantages.Among them,ERCP has high diagnostic specificity and accuracy.It is necessary to choose a reasonable diagnosis method based on the specific conditions of the patient.
