腰硬联合阻滞麻醉对老年下肢骨折患者呼吸功能及肺部感染发生率的影响

摘　　要：目的分析腰硬联合阻滞麻醉对老年下肢骨折患者呼吸功能及肺部感染的影响。方法选取2018年12月至2019年6月本院收治的44例老年下肢骨折患者作为研究对象,随机分为对照组与观察组,各22例。对照组开展全身麻醉,观察组开展腰硬联合阻滞麻醉。比较两组呼吸功能及患者肺部感染情况。结果T2、T3时,观察组RR、PETCO2、SPO2均高于对照组,差异有统计学意义(P<0.05),且观察组呼吸功能变化情况较对照组平稳;观察组肺部感染率低于对照组,但差异无统计学意义。结论老年下肢骨折患者开展腰硬联合阻滞麻醉对患者呼吸功能影响较小,可降低患者术后肺部感染发生率,值得临床推广应用。Objective To analyze the effect of combined spinal-epidural anesthesia on respiratory function and pulmonary infection in elderly pa-tients with lower limb fracture.Methods 44 elderly patients with lower limb fracture treated in our hospital from December 2018 to June 2019 were selected as the research subjects,they were randomly divided into control group and observation group,with 22 cases in each group.The control group was received general anesthesia,and the observation group was received combined spinal-epidural anesthesia.Respiratory function and pulmo-nary infection were compared between the two groups.Results At T2 and T3,RR,PETCO2 and SPO2 in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05),and the change of respiratory function in the observation group was more stable than that in the control group;the pulmonary infection rate of the observation group was lower than that of the control group,but the difference was not statistically significant.Conclusion Combined spinal-epidural anesthesia has little effect on respiratory function in elderly patients with low-er limb fracture,and can reduce the incidence of postoperative pulmonary infection of patients,which is worthy of clinical application.
