摘 要:目的探究血清血管内皮生长因子(VEGF)、同型半胱氨酸(Hcy)与2型糖尿病(T2DM)下肢动脉病变(LEAD)的关系。方法回顾性分析该院2018年1月—2020年12月收治的173例T2DM患者的临床资料,按是否发生下肢动脉病变分为T2DM+LEAD组和T2DM组。另选取同期40名行体检的健康者作对照组(NC)。收集各组临床资料,完善相关实验室检查,采用单因素及Logistic回归模型分析血清VEGF、Hcy与发生下肢动脉病变的关系。结果173例患者中,71例合并下肢动脉病变(T2DM+LEAD组)、102例未合并下肢动脉病变(T2DM组)。T2DM+LEAD组较T2DM组的年龄更高、病程更长,差异有统计学意义(P<0.05)。T2DM+LEAD组、T2DM组、NC组的血清VEGF、Hcy、HbA1c、TG、TC水平均依次降低,ALB水平依次增高,差异有统计学意义(P<0.05)。且T2DM+LEAD组中,A级、B级、C级患者血清VEGF、Hcy水平均依次增高,差异有统计学意义(P<0.05)。Logistic回归分析显示,VEGF(OR=2.273)、Hcy(OR=1.982)、HbA1c(OR=1.799)、ALB(OR=0.401)、TG(OR=3.200)、TC(OR=4.899)、病程(OR=1.550)均是T2DM患者发生LEAD的影响因素。结论血清VEGF、Hcy水平与T2DM患者发生LEAD相关,可作为潜在的早期预测指标。Objective To explore the relationship between serum vascular endothelial growth factor(VEGF),homocysteine(Hcy)and type 2 diabetes(T2DM)lower extremity arterial disease(LEAD).Methods The clinical data of 173 patients with T2DM admitted to the hospital from January 2018 to December 2020 were retrospectively analyzed.According to whether lower extremity arterial disease occurred,they were divided into T2DM+LEAD group and T2DM group.In addition,40 healthy individuals who underwent physical examinations during the same period were selected as the control group(NC).Collected clinical data of each group,improve relevant laboratory examinations,and use single factor and Logistic regression model to analyze the relationship between serum VEGF,Hcy and the occurrence of lower extremity arterial disease.Results Among the 173 patients,71 cases had lower extremity arterial disease(T2DM+LEAD group)and 102 cases did not have lower extremity arterial disease(T2DM group).T2DM+LEAD group was older and longer course of disease than T2DM group,the difference was statistically significant(P<0.05).Serum VEGF,Hcy,HbA1c,TG,TC levels in T2DM+LEAD group,T2DM group,and NC group decreased in sequence,and ALB levels increased in sequence,the difference was statistically significant(P<0.05).In the T2DM+LEAD group,the serum VEGF and Hcy levels of patients with grade A,grade B,and grade C increased sequentially,and the differences between the groups were statistically significant(P<0.05).Logistic regression analysis showed that VEGF(OR=2.273),Hcy(OR=1.982),HbA1c(OR=1.799),ALB(OR=0.401),TG(OR=3.200),TC(OR=4.899),course of disease(OR=1.550)were the influencing factors of LEAD in T2DM patients.Conclusion Serum VEGF and Hcy levels are related to the occurrence of LEAD in T2DM patients,and can be used as potential early predictors.
