银杏叶提取物注射液治疗缺血性脑卒中的快速卫生技术评估

摘　　要：目的:基于快速卫生技术评估(HTA)方法,评价银杏叶提取物注射液治疗缺血性脑卒中的有效性、安全性和经济性,高效、快捷地为临床药物选择和决策者提供循证依据。方法:系统检索中英文数据库和HTA相关网站及数据库。根据纳入和排除标准筛选文章,提取数据,评价文献质量并分析。结果:最终纳入7篇系统评价和4篇经济学研究。在缺血性脑卒中患者中,银杏叶提取物注射液可提高临床疗效,改善神经功能缺损评分,提高日常生活活动能力,对改善血浆黏度、降低纤维蛋白原起到一定作用,还可改善病情恶化率,与对照组相比死亡率差异无统计学意义。不良反应较少,无严重不良反应发生。纳入的经济学研究文献质量较低,实验组和对照组药物疗效均相当,经济学评价均采用成本效果分析法,仅纳入了直接成本中的药品成本,且研究年份相对较早,无法与目前的药品价格相匹配,所得结果难以作为参考。结论:银杏叶提取物注射液治疗缺血性脑卒中具有良好的有效性、安全性。对其经济性的评估尚需进一步开展高质量的药物经济学分析。Objective:To evaluate the effectiveness,safety and economy of Binkgo biloba extract injection in ischemic stroke patients using rapid health technology assessment and to help efficiently and quickly provide evidence-based basis for clinical drug selection and decision makers.Methods:Databases in Chinese and English,and HTA organization websites were searched.Studies were selected according to the inclusion and exclusion criteria,and the data were extracted and the quality of included literature was evaluated and analyzed.Results:At last,7 systematic reviews and 4 pharmacoeconomic studies were included in this study.In patients with ischemic stroke,Ginkgo biloba extract injection improved clinical efficacy,neurological deficit scores and activity of daily living,ameliorated plasma viscosity and reduced fibrinogen,as well as improved the deterioration rate of the disease.There was no statistically significant difference in mortality compared with the control group.Few adverse reactions and no serious adverse reactions were observed.The quality of the included economics research literature was relatively low.Drug efficacy in experimental group and control group was equal.The cost-effectiveness analysis method was adopted in the economic evaluation,in which only the drug cost out of direct cost was included.The year of research is relatively early,which cannot match the current drug price,and results of the researches are difficult to be used as reference.Conclusion:Ginkgo biloba extract injection is an efficacious and safe choice for ischemic stroke treatment.The evaluation of its economics still requires further high-quality pharmacoeconomic analysis.
