中药熏洗辅助甲钴胺片治疗糖尿病周围神经病变临床研究

摘　　要：目的:观察中药熏洗辅助甲钴胺片治疗糖尿病周围神经病变的临床疗效。方法:选取68例糖尿病周围神经病变患者作为研究对象,采用随机数字表法将其分为2组各34例。2组患者均给予基础治疗,对照组在基础治疗上给予甲钴胺片治疗,研究组在对照组治疗方案的基础上加用中药熏洗治疗,观察2组临床疗效。结果:临床疗效总有效率研究组94.12%,高于对照组73.53%,差异有统计学意义(P<0.05)。治疗后,2组症状(疼痛、麻木、乏力)积分、多伦多临床神经病变评分系统(TCSS)评分与治疗前比较均明显降低,研究组症状(疼痛、麻木、乏力)积分、TCSS评分均显著低于对照组,差异均有统计学意义(P<0.05);2组神经传导速度(NCV)包括主侧肢体的正中神经、腓总神经的运动传导速度(MNCV)及感觉传导速度(SNCV)治疗前后比较,差异均有统计学意义(P<0.05);且研究组以上各传导速度均快于对照组(P<0.05)。结论:中药熏洗辅助甲钴胺片治疗可显著改善糖尿病周围神经病变临床症状、神经病变及神经传导速度,提高临床疗效。Objective:To observe the clinical effect of Chinese medicine fumigation and washing assisting mecobalamin tablets for diabetic peripheral neuropathy.Methods:A total of 68 cases of patients with diabetic peripheral neuropathy were selected as the research objects,and were divided into two groups according to the random number table method,34 cases in each group.Both groups were given basic treatment;the control group was additionally given mecobalamin tablets,and the study group was additionally treated with Chinese medicine fumigation and washing based on the treatment of the control group.The clinical effect in the two groups was observed.Results:The total clinical effective rate was 94.12%in the study group,higher than that of 73.53%in the control group,the difference being significant(P<0.05).After treatment,the scores of symptoms like pain,numbness and fatigue as well as Toronto Clinical Scoring System(TCSS)scores in the two groups were significantly decreased when compared with those before treatment,and the above scores in the study group were significantly lower than those in the control group,differences being significant(P<0.05).Before and after treatment,there were significant differences being found in the comparison of the nerve conduction velocity(NCV)including motor conduction velocity(MNCV)and sensory conduction velocity(SNCV)of median nerve and common peroneal nerve of the main limbs between the two groups(P<0.05);the above conduction velocities in the study group were better than those in the control group(P<0.05).Conclusion:The therapy of Chinese medicine fumigation and washing assisting mecobalamin tablets for diabetic peripheral neuropathy can improve the clinical symptoms,nervous lesions and nerve conduction velocity,as well as the clinical effect.
