宫颈扩张棒与卡前列甲酯栓在宫颈手术史绝经后妇女取环中的临床效果观察

摘　　要：目的评价宫颈扩张棒与卡前列甲酯栓在宫颈手术史绝经后妇女取环中的安全性和有效性。方法选择2019年10月至2020年10月我院计划生育门诊行择期B超引导下取环术的绝经后女性共124例,均有宫颈手术史,采用随机生成数字法将其分为扩张棒组和卡前列甲酯栓组各62例。比较两组宫颈软化度、一次性取环成功率、术中和术后出血率和出血量、平均手术时间、术后30 min疼痛视觉模拟量表(VAS)评分以及宫颈损伤发生率。结果卡前列甲酯栓组宫颈软化优良率高于扩张棒组,出血率、出血量和VAS评分低于扩张棒组(P<0.05),两组取环成功率和手术时间比较差异无统计学意义(P>0.05)。两组宫颈损伤发生率均较低,均未出现严重不良反应。结论宫颈扩张棒与卡前列甲酯栓在宫颈手术史绝经后妇女取环中均有广泛应用,卡前列甲酯栓在提高宫颈软化度、降低出血风险和术后疼痛体验方面表现更优越,宫颈损伤发生率较低,安全性较好。Objective To evaluate the safety and efficacy of cervical dilator rod and carboprost suppository for removing intrauterine device(IUD)in postmenopausal women with history of cervical surgery.Methods A total of 124 postmenopausal women visiting the birth-control clinic of our hospital from October 2019 to October 2020 for elective B-ultrasound-guided IUD removal were selected.All patients had cervical surgery history.The patients were randomly divided into dilator group and carboprost group,62 in each group.The degree of cervical softening,success rate of one-time removal,intraoperative and postoperative bleeding rate and volume,mean operative time,pain visual analogue scale(VAS)score after 30 minsof operation and incidence of cervical injury were compared between the two groups.Results The rate of cervical softening was higher while bleeding rate,bleeding volume and VAS score were lower in the carboprost group than those in the dilator group(P<0.05).There were no differences in removal success rate and operation time between the two groups(P>0.05).The incidence of cervical injury was low in both groups and there were no severe adverse reactions.Conclusion Cervical dilator rod and carboprost suppository have been widely used for IUD removal in postmenopausal women with cervical surgery history.Carboprost suppository is betterin improving cervical softening,reducing bleeding risk and postoperative pain experience.It also hasa lower incidence of cervical injury and better safety.
