中西医结合治疗脾胃湿热型脂溢性脱发临床观察

摘　　要：目的:观察木瓜菖蒲三子汤联合西医常规治疗脾胃湿热型脂溢性脱发的临床疗效及对5α-二氢睾酮(DHT)水平的影响。方法:选取符合纳入标准的脾胃湿热型脂溢性脱发患者110例,随机分为两组各55例。对照组予非那雄胺口服及5%米诺地尔外涂治疗,治疗组在对照组基础上予木瓜菖蒲三子汤口服治疗;两组疗程均为6个月。比较两组的临床疗效、症状评分、DHT及游离睾酮(FT)水平。结果:治疗后,治疗组总有效率89.09%高于对照组74.00%,差异有统计学意义(P﹤0.05);治疗组油脂分泌、瘙痒、鳞屑、新发生长症状评分下降(P<0.05或P<0.01),且均优于对照组;治疗前后两组患者的DHT、FT水平比较,差异均不显著。结论:木瓜菖蒲三子汤联合西医常规治疗脾胃湿热型脂溢性脱发临床效果较为显著,可明显改善临床症状,但对抑制DHT、FT血清水平方面与西医常规治疗比较无明显差异。Objective:To observe the clinical efficacy of Mugua Changpu Sanzi decoction on seborrheic alopecia with spleen-stomach damp-heat syndrome and the effect on level of 5α-dihydrostestosterone.Methods:110 cases of selected patients with seborrheic alopecia of spleen-stomach damp-heat type,who met the inclusion criteria,were randomly divided into two groups,55 cases in each group.The control group was treated with oral finasteride tablets and externally applied 5%minoxidil,while the treatment group was treated with Mugua Changpu Sanzi decoction on the basis of control group.Clinical efficacy,symptom score and levels of DHT and free testosterone(FT)were compared between two groups.Results:After 3 months of treatment,5 cases were lost in control group;the total effective rate of treatment group was 89.09%,which was higher than 74.00%of control group,and the difference was statistically significant(P﹤0.05).Compared with before treatment,the symptom scores of oil secretion,itching,scales and new hair growth decreased in treatment group after 6 months of treatment(P﹤0.05 or P﹤0.01),and were better than in control group.The differences of FT and DHT levels of patients between two groups were not significant before and after treatment.Conclusion:The clinical efficacy of Mugua Changpu Sanzi decoction on seborrheic alopecia with spleenstomach damp-heat syndrome is significant,which can obviously improve the clinical symptoms,but not effectively inhibit the serum levels of DHT and FT.
