“正常高值血压”证候分型及相关脉象特征分析

摘　　要：血压水平120~139/80~89 mm Hg(1 mm Hg≈0.133 kPa)定为正常高值血压。通过患者脉象特点和临床表现,将正常高值血压分为脾虚湿盛证、肝肾阴亏证、肾阴阳两虚证、肝郁气滞证。根据临床分型,对正常高值血压的形成、影响因素及脉象进行分析。脾虚湿盛证表现为滑脉和弦滑脉,多见于青壮年、体型肥胖者,以脾胃系统临床表现为著,血压易被忽视已在正常高值范围。肝肾阴亏证多见弦脉、弦数脉,绝经期女性多见,主要由于肝、肾及任脉、太冲脉阴阳失调所致。肾阴阳两虚证多见于先天不足、久病、年老体衰者,表现为弦紧脉,整体脉象沉而无力。肝郁气滞证的出现与性格、情志及脏腑病变等有关,以弦脉、数脉常见,体现在整体以左关脉为代表。正常高值血压对心血管造成不同程度损伤,并增加了心血管疾病的发生率,应发挥中医学未病先防、已病防变特色,积极防控正常高值血压。The blood pressure level of 120~139/80~89 mm Hg(1 mm Hg≈0.133 kPa)is defined as normal high blood pressure.According to the pulse characteristics and clinical manifestations of the patients,the normal high blood pressure was classified into spleen deficiency and dampness excess syndrome,liver and kidney yin deficiency syndrome,kidney yin and yang deficiency syndrome,liver depression and qi stagnation syndrome.According to the clinical classification,the formation of normal high blood pressure,influencing factors and pulse condition were analyzed.The pulse conditions in spleen deficiency and dampness excess syndrome include slippery pulse,wiry and slippery pulse,which are more common in young adults and obese people,especially in the clinical manifestations of spleen and stomach system,and that blood pressure being within the normal high range is easily overlooked.The pulse conditions in liver and kidney yin deficiency syndrome include wiry pulse,wiry and rapid pulse,which are more common in menopausal women,mainly due to the imbalance of yin and yang in the liver,kidney,conception vessel and thoroughfare vessel.Kidney yin and yang deficiency syndrome is related to common in innate deficiency,long illness,the old and weak.Pulse conditions are wiry and tight pulse,and the whole pulse condition is deep and weak.The appearance of liver depression and qi stagnation syndrome is related to personality,emotions and visceral diseases.The common pulse conditions are in wiry pulse and rapid pulse,which is represented by guan pulse of left hand.Normal high blood pressure causes varying degrees of damage to the cardiovascular system,and increases the incidence of cardiovascular diseases.The characteristics of traditional Chinese medicine should be used to prevent before disease onset and control development of existing disease,and actively prevent and control normal high blood pressure.
