清眩汤联合盐酸氟桂利嗪胶囊治疗后循环缺血性眩晕疗效观察

摘　　要：目的探讨清眩汤在后循环缺血性眩晕中的应用效果及对脑血流量的影响。方法选取山东中医药大学附属医院2017年9月-2019年7月收治的后循环缺血性眩晕患者60例,按照随机数字表法分为2组,对照组30例给予盐酸氟桂利嗪胶囊治疗,治疗组30例给予盐酸氟桂利嗪胶囊联合清眩汤治疗,治疗2周后比较2组临床疗效、中医证候疗效,观察治疗前后脑血流速度及搏动指数。结果治疗组临床疗效总有效率为83.3%(25/30),对照组为60.0%(18/30),治疗组显著高于对照组(P<0.05);治疗组中医证候总有效率为86.7%(26/30),对照组为63.3%(19/30),治疗组显著高于对照组(P<0.05)。2组治疗后左椎动脉、右椎动脉、基底动脉血流速度显著增快(P均<0.05);与对照组相比,治疗组治疗后2周后左椎动脉、基底动脉血流速度更快(P均<0.05)。与治疗前相比,2组治疗2周后左椎动脉、基底动脉搏动指数明显降低(P均<0.05);治疗组和对照组治疗2周后PI比较差异均无统计学意义(P均>0.05)。结论在盐酸氟桂利嗪胶囊的基础上合用清眩汤可以增快左椎动脉、基底动脉血流速度,显著提高治疗后循环缺血性眩晕的有效率。Objective It is to explore the effect of Qingxuan Decoction in the treatment of posterior circulation ischemia vertigo and its influence on cerebral blood flow.Methods A total of 60 patients with posterior circulation ischemia vertigo admitted to the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from September 2017 to July 2019 were selected and divided into 2 groups according to the random number table.30 cases in the control group were treated with flunarizine hydrochloride capsules,30 patients in the treatment group were treated with flunarizine hydrochloride capsules combined with Qingxuan Decoction.After 2 weeks of treatment,the clinical efficacy and TCM syndrome efficacy of the two groups were compared,and the cerebral blood flow velocity and pulsation index were observed before and after treatment.Results The total effective rate of clinical efficacy of the treatment group was 83.3%(25/30),and that of the control group was 60.0%(18/30),the treatment group was significantly higher than the control group(P<0.05);the total effective rate of TCM syndromes in the treatment group was 86.7%(26/30),that of the control group was 63.3%(19/30),the treatment group was significantly higher than the control group(P<0.05).The blood flow velocity of the left vertebral artery,right vertebral artery,and basilar artery were increased significantly after treatment in the two groups(all P<0.05);compared with the control group,the blood flow velocity of the left vertebral artery and basilar artery was faster in the treatment group after 2 weeks of treatment(all P<0.05).Compared with before treatment,the pulsatility index of the left vertebral artery and basilar artery in the two groups was significantly reduced after 2 weeks of treatment(P<0.05),there was no significant difference in PI between the treatment group and the control group after 2 weeks of treatment(P>0.05).Conclusion The combination of Qingxuan Decoction on the basis of flunarizine hydrochloride capsules can increase the blood flow vel
