妊娠及妊娠期哮喘对呼出气一氧化氮水平的影响

摘  要:目的探讨妊娠及妊娠期哮喘对呼出气一氧化氮(FeNO)水平的影响。方法选取2018年8月至2020年8月26例健康妊娠女性作为研究一组,20例妊娠期哮喘患者作为研究二组,另选取20例健康非妊娠女性作为对照一组,20例哮喘非妊娠女性作为对照二组,四组均对FeNO水平进行测定,对四组第1秒用力呼气量占预计值百分比(EFV1%)、最大呼气流速峰值占预计值百分比(PEF%)、血嗜酸性粒细胞计数百分比(EOS%)、FeNO进行分析,并对妊娠及妊娠期哮喘不同孕周FeNO水平进行分析,分析健康妊娠观察指标相关性及妊娠期哮喘观察指标相关性。结果对EFV1%、PEF%分析,研究二组、对照二组低于对照一组、研究一组(P<0.05);对EOS%、FeNO水平分析,研究二组、对照二组高于对照一组和研究一组(P<0.05);对研究一组和研究二组不同孕周患者FeNO水平分析,早期、中期及晚期比较,差异无统计学意义(P>0.05);研究一组FeNO水平与EOS%呈正相关(r=0.671,P<0.01);FeNO水平与EFV1%无相关性(P=0.352);FeNO水平与PEF%无相关性(P=0.262)。研究二组FeNO水平与EOS%呈正相关(r=0.670,P<0.01);FeNO水平与EFV1%无相关性(P=0.517);FeNO水平与PEF%无相关性(P=0.301)。结论正常妊娠不会对FeNO水平产生明显影响,而妊娠期哮喘将提高FeNO水平,因此需要予以重视,对于妊娠期哮喘患者可将FeNO作为重要监测指标之一。Objective To analyze the effect of pregnancy and gestational asthma on the level of exhaled nitric oxide(FeNO).Methods A total of 26 cases of healthy pregnant women from August 2018 to August 2020 were selected as the study-one group,and 20 patients with gestational asthma were selected as the study-two group.Another 20 cases of healthy non-pregnant women were selected as the control-one group,and 20 cases of asthma non-pregnancy women were selected as the control-two group.The FeNO levels of four groups were measured.The EFV1%,PEF%,EOS%,and FeNO of the four groups were analyzed.The FeNO levels in different gestational weeks of pregnancy and asthma during pregnancy were analyzed.The correlation between the observed indicators and the healthy pregnancy and asthma during pregnancy was analyzed.Results The forced expiratory volume in 1-second percentage predicted(FEV1%),and the percentage of peak expiratory velocity in the predicted value(PEF%)in the study-two group and control-two group were lower than the control-one group and study-one group(P<0.05).The percentage of eosinophil count(EOS%)and FeNO level in the study-two group and control-two group were higher than those in the control-one group and study-one group(P<0.05).The analysis of the FeNO levels in patients of different gestational ages in the study-one group and study-two group showed there was no statistical difference between the early,middle and late stages(P>0.05).In the study-one group,there was a positive correlation between FeNO level and EOS%(r=0.671,P<0.01).There was no correlation between FeNO level and EFV1%(P=0.352).FeNO level had no correlation with PEF%(P=0.262).There was a positive correlation between FeNO level and EOS%in the study-two group(r=0.670,P<0.01).There was no correlation between FeNO level and EFV1%,P=0.517.There was no correlation between FeNO level and PEF%(P=0.301).Conclusion Normal pregnancy does not have a significant effect on FeNO level,while gestational asthma will improve FeNO level,so attention should be paid to FeNO.
