摘　　要：目的探讨结核菌素皮试(PPD)对结核潜伏感染和预防性服药的影响,并制定其干预策略。方法选择安徽省胸科医院2018年1-6月进行PPD试验并排除活动性结核病者278人,经过PPD检验共发现120例阳性。按照随机数字表法分为两组,每组60例,观察组给予预防性用药,疗程6个月;对照组不用抗结核药物。随访两年,观察两组的肺结核发病率及耐药结核发生率。结果PPD试验阳性率为43.17%。随访的第1年,观察组有3.33%诊断为结核病,而对照组为15.00%,差异有统计学意义(P<0.05);随访第2年两组差异无统计学意义(P>0.05)。随访的2年中,观察组患者的耐药率分别为5.00%和3.33%,差异无统计学意义(P>0.05)。结论PPD可早期发现结核潜伏感染,预防性服药能有效降低活动性结核发生率。Objective To investigate the effect of purified protein derivative test on latent tuberculosis infection and prophylactic medication,and to formulate intervention strategy.Methods A total of 278 persons who underwent PPD test in Anhui Chest Hospital from January to June 2018 and did not have active tuberculosis were enrolled,and 120 cases tested positive by PPD test,were divided into observation group and control group using a random number table,with 60 patients in each group.The patients in the observation group were given prophylactic medication for 6 months,and those in the control group were not given any anti-tubercular agent.The patients were followed up for 2 years to observe the incidence rate of pulmonary tuberculosis and drug-resistant tuberculosis.Results The positive rate of PPD test was 43.17%.In the first year of follow-up,3.33%of the patients in the observation group were diagnosed with tuberculosis,while 15.00%of the patients in the control group were diagnosed with tuberculosis,with a significant difference between the two groups(P<0.05).However,there was no significant difference between the two groups in the second year of follow-up(P>0.05).During the two years of follow-up,the observation group had a drug resistance rate of 5.00%and 3.33%,respectively(P>0.05).Conclusion PPD test can identify latent tuberculosis infection in the early stage,and prophylactic medication can effectively reduce the incidence rate of active tuberculosis.
