Association of Serum BNP and LVEF Levels with Acute ST-Segment Elevation Myocardial Infarction Complicated with Cardiogenic Shock

摘  要:目的探讨左室射血分数(LVEF)、血清B型脑钠肽(BNP)水平与急性ST段抬高型心肌梗死合并心源性休克的关联性。方法选取2018年4月至2019年4月我院急性ST段抬高型心肌梗死合并心源性休克患者42例为心源性休克组,急性ST段抬高型心肌梗死未合并心源性休克患者42例为非心源性休克组,选取同期健康体检者43例为对照组。检测3组LVEF、血清BNP水平并对比,分析LVEF、血清BNP水平与心源性休克的关系。结果3组LVEF、血清BNP水平经单因素方差分析,差异有统计学意义(P<0.05);两两比较,心源性休克组血清BNP水平高于非心源性休克组,LVEF水平低于非心源性休克组,非心源性休克组血清BNP水平高于对照组,LVEF水平低于对照组,差异有统计学意义(P<0.05);经Spearman相关性分析,血清BNP水平与心源性休克呈正相关,LVEF水平与心源性休克呈负相关(P<0.05)。结论急性ST段抬高型心肌梗死合并心源性休克患者LVEF水平显著降低,血清BNP水平显著升高,临床可联合检测LVEF、血清BNP水平,评估心源性休克风险,以早期干预治疗。Objective To explore the correlation between left ventricular ejection fraction(LVEF)and serum B-type natriuretic peptide(BNP)levels and acute ST-segment elevation myocardial infarction with cardiogenic shock.Methods From April 2018 to April 2019,42 patients with acute ST-segment elevation myocardial infarction and cardiogenic shock were selected as cardiogenic shock group,42 patients with acute ST-segment elevation myocardial infarction without cardiogenic shock were selected as non-cardiogenic shock group,and 43 healthy people were selected as control group.The levels of LVEF and serum BNP in three groups were detected and compared,and the relationship between the levels of LVEF and serum BNP and cardiogenic shock was analyzed.Results The LVEF and serum BNP levels of the three groups were analyzed by one-way analysis of variance,and the differences were statistically significant(P<0.05).The levels of serum BNP in the cardiogenic shock group was higher than that in in non-cardiogenic shock group,and the LVEF in the cardiogenic shock group was lower than that in non-cardiogenic shock group;the levels of serum BNP in non-cardiogenic shock group was higher than that in control group,and the level of LVEF was lower than that in control group.The difference of these above was statistical significance(P<0.05).Spearman correlation analysis showed that serum BNP level was positively correlated with cardiogenic shock,while LVEF level was negatively correlated with cardiogenic shock(P<0.05).Conclusion The levels of LVEF and BNP in serum of patients with acute ST-segment elevation myocardial infarction complicated with cardiogenic shock are significantly lower than those of patients with acute ST-segment elevation myocardial infarction.The levels of LVEF and BNP in serum can be detected jointly in order to assess the risk of cardiogenic shock and to intervene early.
