小剂量糖皮质激素联合多索茶碱治疗老年哮喘的效果

摘　　要：目的探讨小剂量糖皮质激素联合多索茶碱治疗老年哮喘的效果。方法前瞻选取2019年5月至2020年5月于我院接受治疗的60例老年BA患者作为研究对象,按随机数字表法分为两组,每组30例。对照组采用小剂量糖皮质激素(布地奈德)治疗,观察组在对照组基础上加用多索茶碱治疗,比较两组治疗前、治疗2周结束时肺功能一秒用力呼气容积(FEV1)、最大呼气峰流速(PEF)、动脉血气[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))及不良反应发生情况。结果治疗2周结束时,两组FEV1、PEF水平均较治疗前升高,且观察组高于对照组(P<0.05);治疗2周结束时,两组PaO2水平均较治疗前升高,PaCO_(2)水平均较治疗前降低,且观察组PaO2水平高于对照组,PaCO_(2)水平低于对照组(P<0.05);治疗过程中,两组均未出现明显不良反应。结论小剂量糖皮质激素联合多索茶碱治疗可提高老年哮喘患者肺功能,改善动脉血气,且不会增加不良反应,用药安全性较高。Objective To investigate the effect of low-dose glucocorticoid combined with doxofylline in the treatment of senile asthma.Methods Prospectively selected 60 senile asthma patients who were treated in our hospital from May 2019 to May 2020 as the research objects.According to the random number table method,they were divided into observation group and control group,with 30 cases in each group.The control group was treated with low-dose glucocorticoid,and the observation group was treated with doxofylline on the basis of the control group.The lung function of the two groups was compared before treatment and at the end of 2 weeks of treatment,maximum peak expiratory flow rate(PEF),arterial blood gas arterial partial pressure of carbon dioxide(PaCO_(2))and the occurrence of adverse reactions.Results At the end of 2 weeks of treatment,the levels of FEV1 and PEF in the two groups were higher than before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).At the end of 2 weeks of treatment,the PaO_(2) levels of the two groups were higher than that before treatment,and the level of PaCO2 was lower than that before treatment;the PaO2 level of the observation group was higher than that of the control group,and the PaCO2 level was lower than that of the control group(P<0.05).During the treatment,there were no obvious adverse reactions in both groups.Conclusion Low-dose glucocorticoid combined with doxofylline therapy can improve lung function and arterial blood gas in elderly patients with senile asthma,without increasing adverse reactions,and the medication is safer.
