肝素表面处理非球面散光型人工晶状体应用于糖尿病性白内障患者的短期临床效果

摘　　要：目的探讨肝素表面处理非球面散光型人工晶状体应用于糖尿病性白内障患者的短期临床效果。方法选择2017年11月至2019年10月于北京大学首钢医院就诊并确诊为糖尿病性白内障的54例(74眼)患者作为研究对象,按照数字表法将其随机分为试验组(28例)及对照组(26例),各37眼。两组均行白内障超声乳化吸除术,在此基础上试验组联合肝素表面处理非球面散光型人工晶状体植入术,对照组联合非球面人工晶状体植入术。比较两组各时间点的裸眼远视力、最佳矫正远视力、散光值及并发症发生情况;分析试验组晶状体轴位旋转情况。结果术后各随访时间点试验组的裸眼远视力高于对照组,散光值小于对照组(P<0.05);两组的最佳矫正远视力比较,差异无统计学意义(P>0.05)。术后1 d,试验组的房水闪辉发生率低于对照组(P<0.05);其余并发症发生率无统计学意义(P>0.05)。试验组人工晶状体轴位旋转均≤10°,且均未进行二次手术调整轴位。结论肝素表面处理非球面散光型人工晶状体应用于糖尿病性白内障患者的短期临床效果显著,可改善裸眼远视力及散光,且安全性较高。Objective To investigate the short term clinical effect of heparin surface modified aspheric astigmatism intraocular lens applied to diabetic cataract patients.Methods A total of 54 patients(74 eyes)diagnosed with diabetic cataract treated in Peking university Shougang hospital from November 2017 to October 2019 were selected as the research subjects,according to the number table method,the patients were randomly divided into experimental group(28 cases)and control group(26 cases),with 37 eyes in each group.Both groups underwent phacoemulsification of cataract,on this basis,the experimental group was combined with heparin surface modified aspheric astigmatism intraocular lens implantation,and the control group was combined with aspheric intraocular lens implantation.The uncorrected distance vision,best corrected distance vision,astigmatism and complications of the two groups were compared at each time point;and the lens axis rotation in the experimental group was analyzed.Results At each follow-up time point after operation,the uncorrected distance vision of the experimental group were higher than those of the control group,and the astigmatism value were lower than those of the control group(P<0.05);there were no statistically significant differences in the best corrected distance vision between the two groups(P>0.05).One day after operation,the incidence of aqueous humor flash in the experimental group was lower than that in the control group(P<0.05);the incidence of other complications were not statistically significant between the two groups(P>0.05).The intraocular lens axis rotation in the experimental group was≤10°,and no secondary operation was performed to adjust the axis position.Conclusion Heparin surface modified aspheric astigmatism intraocular lens applied to diabetic cataract patients has a significant clinical effect,which can improve the distance vision and astigmatism of the naked eye,and has high safety.
