摘 要:目的:基于出生队列研究,探讨PFASs孕期暴露与乙型肝炎(乙肝)疫苗应答之间的系。方法:从2015年~2018年入院产妇中随机抽取51对母婴作为研究对象,使用超高效液相色谱-串联质谱法仪(UPLC-MS/MS)测定脐带血清PFASs;并采用广义线性模型评估孕期PFASs暴露水平与儿童乙肝疫苗免疫应答之间的关系。结果:孕期PFASs暴露水平与乙肝表面抗原(HBsAg)和E抗原(HBeAg)呈正相关,但与乙肝表面抗体(HBsAb)和核心抗体(HBcAb)呈负相关。然而,仅PFOA(β=0.011,95%CI:0.003,0.020)和PFHxS(β=0.004,95%CI:0.001,0.007)与乙肝表面抗原的相关性存在统计学差异。结论:结果提示,PFASs可能是新生儿乙肝免疫应答反应的环境干扰物,同时孕期可能是新生儿免疫系统发育敏感期。Objective To explore the relationship between PFASs exposure during pregnancy and hepatitis B vaccine response based on a birth cohort study.Method From 2015 to 2018,51 pairs of mothers and infants were randomly selected from the parturients admitted to our hospital as the research objects.UPLC-MS/MS was used to determine serum PFASs from umbilical cord.A generalized linear model was used to evaluate the relationship between PFASs exposure during pregnancy and the immune response to hepatitis B vaccine in children.Results PFASs exposure levels during pregnancy were positively correlated with hepatitis B surface antigen(HBsAg)and E antigen(HBeAg),but negatively correlated with hepatitis B surface antibody(HBsAb)and core antibody(HBcAb).However,only PFOA(β=0.011,95%CI:0.003,0.020)and PFHXS(β=0.004,95%CI:0.001,0.007)showed statistically significant differences in association with HBV surface antigen.Conclusion The results suggest that PFASs may be an environmental disruptor of the immune response to hepatitis B in neonates,and pregnancy may be a sensitive period for the development of the immune system in neonates.
