纤维支气管镜肺泡灌洗联合肺导管注药治疗肺脓肿的临床应用

摘　　要：目的:分析纤维支气管镜(简称纤支镜)肺泡灌洗联合肺导管注药治疗肺脓肿的临床应用效果。方法:选择肺脓肿患者共100例,数字表随机分两组每组50例,对照组的患者给予敏感抗生素抗感染、体位引流咯出脓液治疗,观察组在该基础上增加纤支镜肺泡灌洗联合肺导管注药治疗。比较两组住院时间、治疗前后患者降钙素原以及超敏C-反应蛋白、2周治愈率、4周治愈率和6周治愈率、不良反应。结果:观察组住院时间低于对照组,治疗后患者降钙素原以及超敏C-反应蛋白低于对照组,2周治愈率、4周治愈率和6周治愈率高于对照组,差异有统计学意义(P<0.05)。观察组不良反应低于对照组,差异有统计学意义(P<0.05)。结论:敏感抗生素抗感染、体位引流咯出脓液联合纤支镜肺泡灌洗联合肺导管注药治疗对于肺脓肿的治疗效果确切,可减少抗生素全身使用的不良反应,缩短治疗周期,提高治愈率。Objective to analyze the clinical effect of fiberoptic bronchoscope bronchoalveolar lavage combined with pulmonary catheter injection in the treatment of lung abscess.Method A total of 100 patients with lung abscess in our hospital were selected and randomly divided into two groups:control group(n=50)and control group(n=50).The control group was given anti infection with sensitive antibiotics.On this basis,the observation group was treated with bronchoalveolar lavage with fiberoptic bronchoscope combined with pulmonary catheter injection.The hospitalization time,procalcitonin and hypersensitive C-reactive protein before and after treatment,2-week cure rate,4-week cure rate and 6-week cure rate and adverse reactions were compared between the two groups.Results The hospitalization time of the observation group was lower than that of the control group,the procalcitonin and hypersensitive C-reactive protein after treatment were lower than those of the control group,and the cure rates of 2 weeks,4 weeks and 6 weeks in the observation group were higher than those in the control group(P<0.05).The adverse reactions in the observation group were lower than those in the control group(P<0.05).Conclusion sensitive antibiotics against infection,postural drainage and coughing pus combined with bronchoalveolar lavage combined with pulmonary catheter injection are effective in the treatment of pulmonary abscess,which can reduce the adverse reactions of systemic use of antibiotics,shorten the treatment cycle and improve the cure rate.
