NT-proBNP水平与CVP监测对慢性阻塞性肺疾病机械通气患者撤机成功率的影响

摘　　要：目的:探讨对慢性阻塞性肺疾病(COPD)行机械通气治疗的患者,检测N端脑钠肽前体(NT-proBNP)与中心静脉压(CVP)监测对患者撤机成功率的影响。方法:选取收治的行机械通气治疗的60例COPD患者为研究对象,根据患者NT-proBNP、CVP水平分组,其中NT-proBNP≤500 pg/ml,CVP≤15 cm H 2O的30例患者为试验组,NT-proBNP>500 pg/ml,CVP>15 cm H 2O的30例患者为对照组,比较两组患者撤机成功率及患者撤机后各临床指标情况。结果:试验组患者撤机成功率为90.00%,显著高于对照组63.33%的成功率,差异有统计学意义(P<0.05);试验组患者撤机后NT-proBNP、CVP显著低于对照组,FVC显著高于对照组,差异有统计学意义(P<0.05)。结论:COPD行机械通气者,治疗期间进行NT-proBNP水平与CVP的监测,可对患者撤机结局提供一定的预测,提高患者撤机成功率,改善预后。Objective To investigate the effect of monitoring n-terminal brain natriuretic peptide precursor(NT-probNP)and central venous pressure(CVP)on the success rate of withdrawal of patients with chronic obstructive pulmonary disease(COPD)treated with mechanical ventilation.Methods 60 cases of COPD patients with mechanical ventilation treatment were selected as the research object,according to patients'NT-proBNP,CVP level grouping,the NT-proBNP acuities were less than or equal 500 pg/ml,CVP 15 or less cm H 2O as experimental group,30 patients of NT-proBNP>500 pg/ml,CVP>15 cm H 2O in the control group,two groups of patients after the success rate and patients pulled machine various clinical indicators were compared.Results The success rate of withdrawal in the experimental group was 90.00%,significantly higher than that in the control group(63.33%)(P<0.05).After withdrawal,NT-proBNP and CVP in the experimental group were significantly lower than those in the control group,and FVC was significantly higher than that in the control group,with statistically significant difference(P<0.05).Conclusion For patients with COPD undergoing mechanical ventilation,monitoring of NT-probNP level and CVP during treatment can provide a certain prediction for the outcome of patients withdrawing from the machine,improve the success rate of patients withdrawing from the machine,and improve the prognosis.
