摘  要:目的探讨高频彩色多普勒超声联合弹性成像技术(UE)在微小乳腺癌及乳腺良性结节诊断中的价值。方法选取150例行超声及病理学检查乳腺结节患者,根据患者病理学检查结果分为乳腺良性结节127例和微小乳腺癌23例,分析两组患者超声图像特征;对比改良乳腺影像报告和数据系统评分(BI-RADS-US);统计联合诊断结果并分析灵敏度、特异度;计算患者收缩期血流峰速(Vmax)、阻力指数(RI)以及弹性应变率(SR)。结果1)病理结果显示良性127例,恶性23例;联合检查结果显示良性108例,恶性42例。联合检查敏感度、特异度分别为88.79%、90.16%,均高于高频彩色超声检查的77.12%、78.16%,差异有统计学意义(P<0.05);2)微小乳腺癌形态不规则、包膜不完整、边界毛刺检出率分别为78.26%、82.61%、65.22%,高于乳腺良性结节的3.94%、2.36%、1.57%,差异有统计学意义(P<0.05);3)乳腺良性结节与微小乳腺癌BI-RADS-US分级比较差异有统计学意义(P<0.05):乳腺良性结节以2~3级为主,恶性结节以4~5级为主;4)微小乳腺癌Vmax、RI结果及SR比值分别为(20.88±5.16)cm/s、(0.99±0.33)、(2.16±0.62),均高于乳腺良性结节的(14.26±3.12)cm/s、(0.59±0.11)、(1.09±0.33),差异有统计学意义(P<0.05)。结论高频彩色多普勒超声联合弹性成像在乳腺良性结节与微小乳腺癌中显示出结节的超声特征,具有较高的敏感度和特异度。Objective To explore the value of high frequency color Doppler ultrasound combined with elastography(UE)in the diagnosis of micro breast cancer and benign breast nodules.Methods 150 cases of lesions were prospectively evaluated by ultrasonography,and then pathological examination was performed.According to the results of pathological examination,they were divided into 127 cases of benign breast nodule and 23 cases of micro breast cancer.The improved breast imaging reporting and data system score(BI-RADS-US)was compared;The sensitivity and specificity of the combined diagnosis were analyzed;the accuracy,sensitivity and specificity of the combined diagnosis results were statistically analyzed;the systolic peak velocity(Vmax),resistance index(RI)and elastic strain rate(SR)were calculated.Results 1)Pathological results showed that 127 cases were benign and 23 cases were malignant,108 cases were benign and 42 cases were malignant.The sensitivity and specificity of combined examination were 88.79%and 90.16%respectively,which were higher than 77.12%and 78.16%of high frequency color Doppler ultrasound examination;2)The detection rates of irregular shape,incomplete capsule and border burr were 78.26%,82.61%and 65.22%respectively,which were higher than 3.94%,2.36%and 1.57%of benign nodules;3)There was significant difference in BI-RADS-US classification between benign breast nodules and micro breast cancer benign breast nodules were mainly 2~3 grade,and malignant nodules were mainly 4~5 grade;4)The results of Vmax,RI and Sr ratio of micro breast cancer were(20.88±5.16)cm/s,(0.99±0.33),(2.16±0.62),significantly higher than those of benign breast nodules(14.26±3.12)cm/s,(0.59±0.11),(1.09±0.33).Conclusion High frequency color ultrasound combined with elastic imaging has a good application effect in breast benign nodule and micro breast cancer.It can distinguish the benign and malignant nodule,show the ultrasonic characteristics of the nodule,and it has high sensitivity and specificity.It is an effective differential diag
