经皮穿针结合石膏外固定治疗桡骨远端骨折临床观察

摘　　要：目的观察传统手法复位经皮穿针结合石膏固定在治疗桡骨远端骨折中的效果。方法回顾2018年7月—2019年6月用手法复位经皮穿针结合石膏外固定治疗桡骨远端骨折的临床疗效。所有患者均采用中医正骨手法闭合复位经皮克氏针结合石膏外固定技术,4~6周内去除石膏功能锻炼,8周内均门诊拔出克氏针。结果4~8周内11例均得到随访;12周内9例随访,骨折均骨性愈合,24周内4例随访;根据Dienst评分,12周后复查患者疗效优7例,良2例;24周后复查疗效均优;治疗过程并发症:4例有克氏针孔处红肿,门诊换药后缓解,6例出现克氏针松动,6周同时去除石膏及克氏针。结论手法复位经皮穿克氏针结合石膏外固定治疗部分桡骨远端骨折具有方法简单、费用低廉、固定牢靠、无二次手术等优点,值得进一步推广和应用。Objective To observe the effect of traditional manipulative reduction and percutaneous pinning fixation combined with plaster fixation in the treatment of distal radius fractures.Methods From July 2018 to June 2019,the clinical efficacy of manual reduction and percutaneous puncture combined with plaster external fixation in the treatment of distal radius fractures was reviewed.All patients were treated with closed reduction of traditional Chinese medicine bone setting technique and percutaneous Kirschner wire combined with plaster external fixation technology.Plaster function exercise was removed within 4-6 weeks,and Kirschner wire was pulled out in outpatient department within 8 weeks.Results 11 patients were followed up within 4-8 weeks.9 patients were followed up within 12 weeks,all fractures healed,and 4 patients were followed up within 24 weeks.According to Dienst score,7 patients were excellent,2 was good,and all patients were excellent after 24 weeks.Complications during treatment:4 cases had redness and swelling of Kirschner pin hole,which was relieved after dressing change in outpatient department,6 cases had Kirschner wire loosening,and plaster and Kirschner wire were removed at the same time in 6 weeks.Conclusion Manual reduction and percutaneous Kirschner wire combined with plaster external fixation in the treatment of partial distal radius fractures has the advantages of simple method,low cost,reliable fixation and no secondary operation,which is worthy of further promotion and application.
