加味四妙散治疗湿热蕴结型痛风性关节炎临床观察

摘　　要：目的通过观察加味四妙散治疗湿热蕴结型痛风性关节炎患者,评价其临床有效性。方法将80例湿热蕴结型痛风性关节炎患者按随机数字表法,将其分为治疗组和对照组,每组40例。对照组口服塞来昔布胶囊;治疗组在对照组基础上口服加味四妙散。比较2组患者治疗后临床疗效,治疗前后症状评分(全身症状、关节疼痛、关节肿胀、活动受限、关节功能)以及血清CRP、ESR、UA水平变化。结果治疗组总有效率为90.00%,高于对照组72.50%;与治疗前相比,治疗后2组患者症状评分(全身症状、关节疼痛、关节肿胀、活动受限、关节功能)及血清CRP、ESR、UA水平均降低,且治疗组较对照组下降明显(均P<0.05)。结论加味四妙散可有效缓解湿热蕴结型痛风性关节炎患者的临床症状、增强关节功能,同时可降低血尿酸及炎症指标,提高总体治疗效果。Objective To observe the effect of modified Simiao powder in the treatment of accumulated dampness-heat gouty arthritis,in order to evaluate the clinical effect.Methods According to the random number table method,80 patients with accumulated dampness-heat gouty arthritis were divided into a treatment group and a control group,with 40 cases in each.The control group took celecoxib capsules orally,0.2 g/time,2 times/d,taken separately in the morning and evening,and the treatment group took orally modified Simiao powder on the basis of the control group,1 dose/d,divided into morning and evening.Both groups took 7 days as a course of treatment,and both groups were treated for 2 consecutive courses.The clinical efficacy of the two groups of patients after treatment,before and after treatment,systemic symptoms,joint pain,joint swelling,limitation of movement,joint function classification,and changes in serum CRP,ESR,and UA levels was compared.Results The effective rate in the control group was 72.50%,which was lower than 90.00%in the treatment group.Compared with before treatment,the systemic symptom score,joint pain score,joint swelling score,activity limitation score,joint function classification score and serum CRP of the two groups were compared after treatment,ESR,UA levels decreased,and the treatment group decreased significantly comparing with the control group(all P<0.05).Conclusion Modified Simiao powder combined with celecoxib shows a positive effect in treating accumulated dampness-heat gouty arthritis,improve the clinical symptoms and joint function of patients,decrease serum uric acid and inflammation index,and improve overall therapeutic effect.
