基于真实世界刺五加注射液治疗1093例情志异常患者的临床应用特征及用药分析

摘　　要：目的探索真实世界中刺五加注射液治疗1 093例情志异常患者的临床应用特征及用药分析。方法基于HIS数据库16家医院中使用的刺五加注射液的1 093例患者的年龄、性别、职业、入院/出院病情、费用等,采用Apriori算法建立模型使用关联规则的方法分析。结果在使用刺五加注射液治疗情志异常的1 093例患者中,女性患者数量最多(70.45%);平均年龄为56.4岁,45~65岁患者数量最多(52.79%);西医诊断疾病以植物神经功能紊乱最多(15.22%);中医诊断证候中,肾虚证占最多(17.31%),其次是气虚血瘀证(10.83%)、气阴两虚证(10.6%)。刺五加注射液联合1种西药和1种中药与联合1种中药和2种西药时的药物类别频数最高的均为祛瘀剂+安定药,其次是祛瘀剂+钙通道阻断药;联合2种中药和1种西药时,药物类别频数最高的是祛瘀剂+清热剂+安定药,其次是祛瘀剂+清热剂+钙通道阻断药。结论使用刺五加注射液的情志异常病患多见于中老年人,且女性明显多于男性,中医证候以肾虚最为多见。刺五加注射液治疗情志异常联合用药广泛,中西药均占相当大的比例,应注重中西药联用,以达最佳的治疗效果。Objective To explore the clinical application characteristics and medication analysis of Acanthopanax Injection in treating 1 093 patients with abnormal emotion. Methods Based on the age, sex, occupation, admission/discharge condition, cost, etc of 1 093 patients with Acanthopanax Injection used in 16 hospitals in HIS database, method analysis of establishing model using association rules based on Apriori Algorithm. Results Among the 1 093 patients treated with Acanthopanax Injection, the number of female patients was the most(70.45%);the average age was 56.4 years old, the number of 45-65 years old patients was the most(52.79%);Most of the diseases diagnosed by Western medicine were autonomic nerve dysfunction(15.22%);among TCM diagnostic syndromes, kidney deficiency syndrome accounted for the most(17.31%), followed by Qi deficiency and blood stasis syndrome(10.83%), Qi and Yin deficiency syndrome(10.6%). The highest frequency of the drug category of the combination of a western medicine and a Chinese medicine is the exfoliant + stabilizer, followed by the exfoliant + calcium channel blocking drug;when combined with a kind of traditional Chinese medicine and two kinds of western medicine, the highest frequency of the drug category is still exfoliant + stabilizer, followed by exfoliant + calcium channel blocking drug;when combined with two kinds of traditional Chinese medicine and one kind of western medicine, the highest frequency of the drug category is exfoliant + heat freshener + stabilizer, followed by exfoliant + heat freshener + calcium channel blocking drug. Conclusion The patients with abnormal emotions who use Acanthopanax Injection are more common in the middle-aged and elderly, and women are significantly more than men. The kidney-deficiency is more common.Acanthopanax Injection is widely used in the treatment of emotional disorder, with a large proportion of both Chinese and western medicines.
