摘　　要：目的分析三水地区老年髋部脆性骨折患者25羟基维生素D水平。方法拟选取于2019年1月—2020年6月在佛山市中医院三水医院住院部收治的老年(年龄大于60岁)骨质疏松症患者200例,依据是否合并髋部脆性骨折(无外伤或轻微外伤情况下引起的骨折)将其分为合并组(n=100)和未合并组(n=100)。所选患者均通过索灵全自动化学发光免疫分析仪对25羟基维生素D水平予以检测,对照分析两组检测结果。结果合并组患者25羟基维生素D水平为(14.87±3.99)ng/mL,其中50例25羟基维生素D缺乏、27例25羟基维生素D不足、23例25羟基维生素D充足;未合并组25羟基维生素D水平为(24.91±3.63)ng/mL,其中30例25羟基维生素D缺乏、28例25羟基维生素D不足、42例25羟基维生素D充足;合并组25羟基维生素D水平低于未合并组,且25羟基维生素D水平缺乏和不足患者占比高于未合并组(P <0.05)。合并组钙离子、镁离子水平、血清磷水平低于未合并组(P <0.05)。结论老年髋部脆性骨折与25羟基维生素D水平之间存在一定联系,警惕低水平25羟基维生素D,可以为临床诊疗效果判断提供一定支持。Objective To analyze the levels of 25 hydroxyvitamin D in senile patients with brittle fracture of hip in Sanshui area.Methods 200 elderly(over 60 years old)osteoporosis patients admitted to the inpatient department of Sanshui Hospital of Foshan Traditional Chinese Medicine hospital from January 2019 to June 2020 were selected,and they were divided into the combined group(n=100)and the non-combined group(n=100)according to whether they had complicated hip brittle fracture(fracture caused by no or minor trauma).All the selected patients were tested by soling automatic chemiluminescence immunoanalyzer for 25 hydroxyvitamin D level,and the test results of the two groups were compared and analyzed.Results The 25-hydroxyvitamin D level in the combined group was(14.87±3.99)ng/mL,including 50 cases of 25-hydroxyvitamin D deficiency,27 cases of 25-hydroxyvitamin D deficiency and 23 cases of 25-hydroxyvitamin D adequate.The 25-hydroxyvitamin D level in the uncombined group was(24.91±3.63)ng/mL,including 30 cases of 25-hydroxyvitamin D deficiency,28 cases of 25-hydroxyvitamin D deficiency,and 42 cases of 25-hydroxyvitamin D adequate.The level of 25 hydroxyvitamin D in the combined group was lower than that in the noncombined group,and the proportion of patients with 25 hydroxyvitamin D deficiency and deficiency was higher than that in the non-combined group(P<0.05).The levels of calcium and magnesium ions and serum phosphorus in the combined group were lower than those in the non-combined group(P<0.05).Conclusion There is a certain correlation between senile hip brittle fracture and 25 hydroxyvitamin D level,so vigilance against low 25 hydroxyvitamin D level can provide certain support for clinical diagnosis and treatment effect judgment.
