宫颈扩张球囊联合催产素对足月妊娠引产产妇的临床效果

摘　　要：目的探讨宫颈扩张球囊联合催产素对足月妊娠引产产妇的临床效果。方法50例足月妊娠引产产妇随机均分为对照组(催产素催产)和观察组(催产素联合宫颈扩张球囊催产),采用Bishop评分评估2组产妇宫颈的成熟情况及效果;分别于分娩前和分娩后采集2组产妇空腹静脉血3 mL,采用酶联免疫吸附法检测血清中皮质醇(CORT)、儿茶酚胺(CA)及谷氨酸(GLU)的含量;记录2组产妇分娩情况(诱发临产时间、剖宫产率、引产成功率及引产成功率效果)和产后并发症。结果观察组产妇Bishop评分和促宫颈成熟率分别高于对照组(P<0.05或P<0.01),诱发临产时间、剖宫产率及产后并发症发生率均小于对照组(P<0.05或P<0.001),且引产成功率高于对照组(P<0.05);2组产妇分娩后CORT、CA及GLU水平均较分娩前下降,且观察组产妇均低于对照组(P<0.05或P<0.01)。结论与单纯催产素催产方式相比,宫颈扩张球囊联合催产素可以更好地改善足月妊娠引产产妇的宫颈成熟度,降低分娩应激反应,提高引产成功率及降低产后并发症。Objective To explore the clinical effect of cervical dilated balloon and oxytocin on maternal induction in term pregnancy.Methods Fifty women with term pregnancy were randomly divided into control groups(oxytocin)and observation groups(oxytocin combined with cervical dilation balloon oxytocin).Bishop score was used to evaluate the cervical maturity and effect of 2 women.Two groups of 3 mL maternal fasting venous blood were collected before and after delivery,and the content of serum cortisol(CORT),catecholamine(CA)and glutamate(GLU)were detected by enzyme-linked immunosorption method.The delivery situation of 2 maternal groups(induced temporary delivery time,cesarean section rate,labor introduction success rate and labor introduction success rate)and postpartum complications were recorded.Results Maternal Bishop score and cervical maturity rate were higher than in the control group(P<0.05 or P<0.01).Induced labor time,cesarean section rate and postpartum complications were all less than the control group(P<0.05 or P<0.001),and the labor induction success rate was higher than that in the control group(P<0.05).The CORT,CA,and GLU levels were lower compared with before delivery,and the observed women were lower than the control group(P<0.05 or P<0.01).Conclusion Compared with the simple oxytocin method,cervical dilation balloon combined with oxytocin can better improve the cervical maturity of pregnant women in term pregnancy,reduce the delivery stress response,improve the success rate of labor induction,and reduce postpartum complications.
