眼针联合柴胡疏肝汤治疗肝气郁结型中风后抑郁的临床研究

摘　　要：目的探讨眼针联合柴胡疏肝汤治疗肝气郁结型中风后抑郁的疗效及血清5-羟色胺(5-hydroxytryptamine, 5-HT)、去甲肾上腺素(noradrenaline, NE)的影响。方法将2016年3月—2019年12月期间134例肝气郁结型缺血性中风后抑郁患者,随机分两组,各67例,对照组采用氟西汀20 mg/d治疗;在此基础上,观察组采用眼针联合柴胡疏肝汤治疗,持续治疗4周。分别评估患者的汉密尔顿抑郁量表(hamilton depression scale, HAMD)分级,治疗前、治疗2周和治疗4周时测定血清5-HT、NE水平,比较两组患者的临床疗效。结果与治疗前比,两组患者治疗后HAMD抑郁程度均改善;与对照组比,观察组患者治疗后HAMD抑郁程度改善更大,有统计学意义(P<0.05)。与治疗前比,两组患者治疗2周、治疗4周血清5-HT、NE水平升高;与对照组比,观察组患者治疗2周、治疗4周血清5-HT、NE水平较高,有统计学意义(P<0.05)。对照组有效率85.07%(57/67)低于观察组有效率95.52%(64/67),有统计学意义(P<0.05)。结论眼针联合柴胡疏肝汤治疗肝气郁结型中风后抑郁患者的疗效确切,有助于降低抑郁分级,提高5-HT、NE水平。Objective To explore the clinical effect of Chaihu Shugan Decoction(柴胡疏肝汤) for relieving liver Qi combined with eye acupuncture in the treatment of depression after apoplexy of stagnation of liver Qi, and the effects of levels of 5-hydroxytryptamine(5-HT) and noradrenaline(NE) in serum. Methods A total of 134 patients with depression after ischemic stroke of stagnation of liver Qi from March 2016 to December 2019 were randomly divided into two groups, 67 cases in each group, and the control group received fluoxetine 20 mg. The observation group additionally received eye acupuncture combined with Chaihu Shugan Decoction for 4 weeks. The levels of serum 5-HT and NE were measured before treatment, 2 and 4 weeks after treatment. Results Compared with those before treatment, the levels of HAMD depression in the two groups were improved after treatment. Compared with those of the control group, the levels of HAMD depression in the observation group were improved significantly after treatment(P<0.05). Compared with those before treatment, the levels of serum 5-HT and NE in two groups were increased in the second and fourth week of treatment. Compared with those of the control group, the levels of serum 5-HT and NE of the observation group were higher in the second and fourth week of treatment(P<0.05). The effective rate of the control group(85.07%,57/67) was lower than that of the observation group(95.52%,64/67)(P<0.05). Conclusion Chaihu Shugan Decoction for relieving liver Qi combined with eye acupuncture is effective in the treatment of post-stroke depression of liver Qi stagnation type, which is helpful to reduce the depression grade and improve the levels of 5-HT and NE.
