摘 要:目的通过检测血清8-iso-PGF2α水平验证氧化应激在2型糖尿病(type 2 diabetes, T2DM)大血管病变中的作用,并明确芪归药对对气虚血瘀型T2DM大血管病变患者的作用。方法选取气虚血瘀型T2DM大血管病变患者70例随机分为对照组、芪归5∶1组及芪归3∶2组,另选取门诊查体健康成人与单纯T2DM患者各25例作为空白组与单纯T2DM组;单纯T2DM组与对照组维持原有治疗方案不变,芪归5∶1组与芪归3∶2组在原治疗方案基础上分别加用不同配比的芪归中药方剂,治疗90 d后重新测定指标,比较治疗前后各指标变化情况。结果治疗前对照组、芪归5∶1组及芪归3∶2组血清8-iso-PGF2α与相关指标水平明显高于空白组与单纯T2DM组(P<0.05);治疗后芪归5∶1组与芪归3∶2组相关指标及血清8-iso-PGF2α水平较治疗前均有不同程度下降(P<0.05)。结论氧化应激在T2DM大血管病变发生发展过程中发挥一定作用;芪归药对能改善气虚血瘀型T2DM大血管病变患者的相关指标及血清8-iso-PGF2α水平,或与其抵抗氧化应激机制有关。Objective To verify the role of oxidative stress in the pathogenesis of macroangiopathy due to T2 DM by detecting the level of serum 8-iso-PGF2α and clarify the effect of Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix) on the T2 DM patients with macroangiopathy of Qi deficiency and blood stasis type. Methods Seventy T2 DM patients with macroangiopathy of Qi deficiency and blood stasis type were randomly divided into control group, Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix) 5∶1 group and Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix) 3∶2 group. And 25 healthy people were selected as the blank group and 25 outpatients only with T2 DM were as the simple T2 DM group. The simple T2 DM group and control group maintained their original treatment plan unchanged. Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix) 5∶1 and 3∶2 groups added different doses of Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix) on the basis of original treatment plan, and we re-measured indexes after 90 days of treatment and compares the indicators to those before treatment. Results Before treatment, the levels of serum 8-iso-PGF2α and related parameters in the control group, Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix) 5∶1 and 3∶2 groups were significantly higher than those in the blank group and simple T2 DM group(P<0.05). Compared with those before treatment, the levels of 8-iso-PGF2α and related parameters in Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix) 5∶1 and 3∶2 groups were decreased in different degrees after treatment(P<0.05). Conclusion Oxidative stress plays a role in the pathogenesis of T2 DM macrovascular lesions, and the prescription of Huangqi(Astragali Radix)-Danggui(Angelicae Sinensis Radix) can improve the levels of related parameters and 8-iso-PGF2α in T2 DM patients with macrovascular lesions with Qi deficiency and blood stasis, and perhaps it resists oxidative stress.
