天麻钩藤颗粒联合俞募配穴埋线治疗颈源性高血压临床观察

摘　　要：目的观察天麻钩藤颗粒联合俞募配穴埋线治疗颈源性高血压的临床疗效。方法选取医院门诊2019年1月—2020年1月,临床诊断颈源性高血压的患者120例,分为四组,对照组、天麻钩藤饮组、埋线治疗组、综合治疗组,治疗周期为4周,观察周期为8周。在第4周、第8周分别对比四组患者治疗前后的血压变化及颈部NDI评分、VAS评分。结果试验前后所有患者均完善常规检查及颈椎间盘CT平扫。四组患者比较,试验组与对照组用药前临床评价无差别(P>0.05);治疗第4周,四组治疗颈源性高血压均可以明显降低患者的血压,但是对照组未改善颈部NDI评分、VAS评分;其余三组均能同时明显改善颈部评分;治疗第8周,对照组血压出现反复,其他三组血压、颈部NDI评分、VAS评分、中医证候积分无明显反复,与对照组降压效果比较,天麻钩藤饮组为(P<0.05)、埋线治疗组(P<0.05)、综合治疗组(P<0.01)。结论天麻钩藤颗粒联合俞募配穴埋线治疗颈源性高血压临床效果确定,在降低颈源性高血压的同时能够显著改善颈部功能及不适,体现了中医治疗疾病的独特和长效优势,值得临床应用。Objective To observe the clinical effect of Tianma Gouteng Granule(天麻钩藤颗粒) combined with Shu-Mu acupoints catgut embedding in the treatment of cervical hypertension. Method According to the patients’ visit number,a randomized parallel control method was used to select 120 patients with clinically diagnosed cervical hypertension from the outpatient department of traditional Chinese medicine of our hospital from January 2019 to January 2020. According to the wishes of the patients,they were divided into four groups: the control group,Tianma Gouteng Granule group,catgut embedding treatment group and comprehensive treatment group( oral Tianma Gouteng Granule combined with Shu-Mu acupointscatgut embedding treatment),with a treatment period of 4 weeks. The cycle was 8 weeks. The changes of blood pressure,neck NDI score and VAS score before and after treatment were compared in the fourth and eighth week. Results Routine examination and plain CT scan of cervical intervertebral disc were completed in all patients before and after the test. Compared with the four groups,there was no difference in clinical evaluation between the experimental group and the control group( P >0. 05). In the fourth week of treatment,the four groups could significantly reduce the blood pressure of patients with cervical hypertension,but the control group did not improve the neck NDI score or VAS score. The other three groups could significantly improve the neck score at the same time. At the 8 th week of treatment,the blood pressure in the control group was repeated,but there was no significant repetition in the other three groups,such as blood pressure,neck NDI score,VAS score and TCM syndrome score.Compared with the control group,Tianma Gouteng Granule group was significant( P< 0. 05),catgut embedding groupwas significant( P< 0. 05) and comprehensive treatment group was more significant( P< 0. 01).
